Objective:OurobjectivewastocharacterizeprostatebiopsyoutcomesandtrendsinChinesemenovera10-yearperiod, sincetheincreasinguseofPSAtests. Methods:Allmen(n=1,650)whounderwentprostatebiopsyforPCaatHuashanHospital,Shanghai,Chinafrom2003–2011 were evaluated. Demographic and clinical information was collected for each patient, including age, digital rectal examination (DRE), transrectal ultrasound (prostate volume and nodule), total prostate-specific antigen (tPSA) levels and freePSAratio(fPSA/tPSA)priortobiopsy.ProstatebiopsywasperformedusingsixcoresbeforeOctober2007ortencores thereafter.Logisticregressionandmultivariateanalysiswereusedtoevaluateourdata. Results:TheoverallpositiverateofprostatebiopsyforPCawas47%andtheratedecreasedsignificantlyovertheyears from74%in2003to33%in2011(P-trend=0.004).Ageatdiagnosiswasslightlyincreased(P-trend=0.04)whilefPSA/tPSA wassignificantlydecreased(P-trend=1.11610-5).AstatisticallysignificanttrendwasnotobservedfortPSAlevels,prostate volume,orproportionofpositivenodule.Themodelincludingmultipledemographicandclinicalvariables(i.e.,age,DRE, tPSA,fPSA/tPSAandtransrectalultrasoundresults)(AUC=0.93)statisticallyoutperformedmodelsthatincludedonlyPSA (AUC=0.85)orfPSA/tPSA(AUC=0.66)topredictPCarisks(P,0.05).Similarresultswereobservedinasubgroupofmen whosetPSAlevelswerelowerthan
