WeaimedtodetermineifMALAdiffersinoutcomefromLAofotherorigin(LAOO). Methods:WeconductedaretrospectiveanalysisofallpatientsadmittedwithLAtoourmedicalICUofatertiary referralcenterduringa5-yearperiod.MALApatientsandLAOOpatientswerecomparedwithrespectto parametersofacid-basebalance,serumcreatinine,hospitaloutcome,SimplifiedAcutePhysiologyScoreII(SAPSII) andSequentialOrganFailureAssessment(SOFA)score,usingPearson’sChi-squareortheMann-WhitneyU-test. Results:Of197patientsadmittedwithLA,10hadbeendiagnosedwithMALA.WithMALA,medianarterialblood pHwassignificantlylower(6.78[range6.5to6.94])andserumlactatesignificantlyhigher(18.7±5.3mmol/L)than withLAOO(pH7.20[range6.46to7.35],meanserumlactate11.2±6.1mmol/L).Overallmortality,however,was comparable(MALA50%,LAOO74%).Furthermore,survivalofpatientswitharterialbloodpH7.00(N=41)was significantlybetter(50%vs.0%)ifMALA(N=10)wastheunderlyingconditioncomparedtoLAOO(N=31). Conclusions:Comparedtosimilarlyseverelacticacidosisofotherorigin,theprognosisofMALAissignificantly better.MALAshouldbeconsideredinmetformin-treatedpatientspresentingwithlacticacidosis.
