Methods:Todate,266patientshavebeenenrolledinGENISOS,aprospective,observationalcohortofpatients withearlysystemicsclerosis.Inadditiontopulmonaryfunctiontests(PFTs),clinicalandlaboratorydatawere obtainedfromeachpatient.Weanalyzed926FVCmeasurementsutilizinggeneralizedlinearmixedmodels.The predictivesignificanceofbaselinevariablesforthedeclinerateinFVCwasinvestigatedbytheinteractionterm betweenthevariableandthefollow-uptimewithinthefirst3yearsafterenrollmentaswellasthroughoutthe entirefollow-uptime. Results:Thecohortconsistedof125white,54AfricanAmerican,and77Hispanicpatientswithaveragedisease durationof2.5yearsatenrollment.Themeanfollow-uptimewas3.8years,rangingupto11.4years.Anumberof baselinevariables,includingantibodystatus,AfricanAmericanethnicity,diseasetype,baselinePFTvalues,modified RodnanSkinScore,fibrosisonchestradiograph,andlungandskinsubscoresoftheSeverityIndex,wereassociated withseriallymeasuredFVClevels.However,onlythepresenceofanti-topoisomeraseIantibodies(ATA)was associatedwithlowerFVClevels(P0.001)aswellasaccelerateddeclinerateinFVCwithinthefirst3yearsof follow-up(P=0.02).NoneofthebaselinevariablespredictedtherateofdeclineinFVConlong-termfollow-up.
