Methods:Theprevalenceandcomorbidityofdiabetesmellituswereanalyzedinanationallyrepresentativesample (N=9133)ofthenon-institutionalizedGermanadultpopulationaged50yearsandolder.Informationonphysician- diagnoseddiabetesand20otherchronichealthconditionswascollectedaspartofthenationaltelephonehealth interviewsurvey‘GermanHealthUpdate(GEDA)’2009.Overall,51.2%ofcontactedpersonsparticipated.Among personswithdiabetes,diabetesseveritywasdefinedaccordingtothetypeandnumberofdiabetes-concordant conditions:nodiabetes-concordantcondition(grade1);hypertensionand/orhyperlipidemiaonly(grade2);one comorbiditylikelytorepresentdiabetes-relatedmicro-ormacrovascularend-organdamage(grade3);severalsuch comorbidities(grade4).Determinantsofdiabetesseveritywereanalyzedbymultivariableordinalregression. Results:The12-monthprevalenceofdiabeteswas13.6%withnosignificantdifferencebetweenmenandwomen. Personswithdiabeteshadasignificantlyhigherprevalenceandaveragenumberofdiabetes-concordantaswellas diabetes-discordantcomorbiditiesthanpersonswithoutdiabetes.Amongpersonswithdiabetes,10.2%,46.8%, 35.6%and7.4%wereclassifiedashavingseveritygrade1–4,respectively.Determinantsofdiabetesseverity includedage(cumulativeoddsratio1.05,95%confidenceinterval1.03-1.07,peryear)andnumberofdiscordant comorbidities(1.40,1.25-1.55).Withrespecttospecificdiscordantcomorbidities,diabetesseveritywascorrelatedto depression(2.15,1.29-3.56),respiratorydisease(2.75,1.72-4.41),musculoskeletaldisease(1.53,1.06-2.21),andsevere hearingimpairment(3.00,1.21-7.41).
