Juki?etal.DiagnosticPathology2011,6:5 /content/6/1/5 CASE REPORT OpenAccess Primaryacantholyticsquamouscellcarcinomaof thececum:acasereport ZoranJuki?1,IvaLedinsky2,MonikaUlamec3,MarioLedinsky2,Bo?oKru?lin3,4,DavorTomas3,4* Abstract Background:Acantholyticsquamouscellcarcinoma(ASCC)isanuncommonhistopathologicvariantofSCC, characterizedbymarkedacantholysis,whereinthetumorcellsdemonstratedefectivecohesiontooneanotherin thecancernestleadingtoapseudoglandularorpseudovascularappearance.ThemostcommonsiteofASCCis thesun-exposedareasoftheskin.SporadiccasesofASCChavealsobeenreportedinvariousmucosalmembranes andorgansbuttoourknowledgethisisthefirstcaseofprimaryASCCofthelargebowel. Casepresentation:A59-year-oldwomanunderwentrighthemicolectomyduetolargetumorincecumand initialpartoftheascendingcolon.Microscopically,thetumorconsistedofnestsoffocallykeratinizinglarge, atypical,squamousepithelialcells.Approximately70%ofthetumorshowedacantholyticchangesandacantholysis wasequallydistributedthroughtheentiretumor.Immunohistochemicallytumorcellswerediffuselypositivefor cytokeratin(CK)AE1/AE3andfocallypositiveforepithelialmembraneantigenandsyndecan1.Allothertested antibodies(CK7,CK20,CKMNF116,E-cadherin,beta-catenin,p63,p16,CD31,CD34,CEA,estrogen,progesterone) showednegativereaction.PeriodicacidSchiffandalcianbluestainingshowednointracellularorextracellular mucinousmaterialinthetumor.Thediagnosisofacantholyticsquamouscellcarcinomaofthececumwas suspectedandadditionalexaminationwasrecommendedtoexcludepossibilityofmetastaticcarcinoma.Extensive clinicalexaminationwhichalsoincludedwhole-bodyPET/CTscanshowednoadditionaltumors.Afterthe exclusionofpossiblemetastaticdiseasethediagnosisofprimaryacantholyticsquamouscellcarcinomaofthe cecumwasconfirmed.Sixmonthsaftersurgerythemetastasisinsmallintestineandrecurrenceintheabdominal cavityatthesiteofsurgeryappearedandhadthesamemorphologicalcharacteristicastheprimarytumorinthe cecum. Conclusion:WereportauniquecaseofASCCarisingincecumandonthiswayexpands
