Methodology/Principal Findings: We evaluated the associations between preoperative serum PGRN levels and breast cancerrecurrenceinacohortof697newlydiagnosedbreastcancerpatientswhounderwentcurativesurgerybetweenApril 2001 and December 2004. The mean age 6 standard deviation (SD) was 4669.8 years, and all patients with hormone receptor(HR)-positivetumorsreceivedadjuvanttamoxifentherapy.Atamedianfollow-upof62.2months(range,2.9–98.2), 89patients(12.8%)hadexperiencedarecurrenceand51patients(7.3%)haddied.IntheHR-positivegroup,serumPGRN levels were associated with recurrence according to the log-rank test for trend (p for trend =0.049). There was no association between PGRN levelsand recurrence in the HR-negative group (p for trend =0.658). Adjusted hazard ratios, includingpossibleconfounders,revealedalinearrelationshipbetweenserumPGRNlevelsandrecurrenceintheHR-positive group(pfortrend =0.049),andthisassociationwasfurtherstrengthenedafterexcludingpatientswhohadnolymphnode metastasis(pfortrend =0.038). Conclusions/Significance: Serum PGRN levels were clinically significant for predicting recurrence in patients with HR- positivebreastcancerduringadjuvanttamoxifentherapy. Citat
