Methods:UsingdatafromamulticentercohortstudyconductedinFrance,Germany,andtheUnited-Kingdom (EuroSC),weperformedCoxproportional-hazardsmodelstoestimatetheassociationsbetweenQoLatbaseline andtheoccurrenceofrelapseovera24-monthperiod,withadjustmentforage;gender;positive,negativeand generalpsychopathologyPANSSfactors;functioning(GAF);medication;side-effects;andcompliancemeasures. Results:Oursampleconsistedof1,024patients;540(53%)hadatleastoneperiodofrelapse,and484(47%)had norelapse.QoLlevelswerethemostimportantfeaturespredictingrelapse.WefoundthatahigherlevelofQoL predictsalowerrateofrelapseat24months:HR=0.82(0.74;0.91),p0.001fortheSF36-PhysicalCompositeScore; andHR=0.88(0.81;0.96),p=0.002fortheSF36-MentalCompositeScore.Theseresultswerenotconfirmedusing theQoLI:HR=0.91(0.81;1.01),p=0.083.Toalesserextent,olderage,betterfunctioning,andahighercompliance scorealsopredictalowerrateofrelapseat24months(HRsfrom0.97to0.98;p0.05). Conclusions:QoL,asassessedbytheSF36,isanindependentpredictorofrelapseata24-monthfollow-upin schizophrenia.ThisfindingmayhaveimplicationsforfutureuseoftheQoLinpsychiatry.Moreover,ourfindings maysupportthedevelopmentandmonitoringofcomplementarytherapeuticapproaches,suchas‘recovery- oriented’combinedwithtraditionalmentalhealthcarestopreventrelapse.
