VeryfewmolecularmethodsfordetectionofGASinclinicalthroatswabspecimenshavebeendescribed. Methods:Weperformedastudyofalaboratory-developedinternally-controlledrapidGroupAstreptococcus (GAS)PCRassayusingflockedswabthroatspecimens.WecomparedtheGASPCRassaytoGAScultureresults usingacollectionofarchivedthroatswabsamplesobtainedduringastudycomparingtheperformanceof conventionalandflockedthroatswabs. Results:ThesensitivityoftheGASPCRassayascomparedtothereferencestandardwas96.0%(95%CI90.1%to 98.4%),specificity98.6%(95%CI95.8%to99.5%),positivepredictivevalue(PPV)96.9%(95%CI91.4%to99.0%)and negativepredictivevalue(NPV)of98.1%(95%CI95.2%to99.2%).Forconventionalswabcultures,sensitivitywas 96.0%(95%CI90.1%to98.4%),specificity100%(95%CI98.2%to100%),PPV100%,(95%CI96.1%to100%)and NPV98.1%(95%CI95.2%to99.3%) Conclusions:Inthisretrospectivestudy,theGASPCRassayappearedtoperformaswellasconventionalthroat swabculture,thecurrentstandardofpractice.SincetheGASPCRassay,includingDNAextraction,canbe performedinapproximately1hour,prospectivestudiesofthisassayarewarrantedtoevaluatetheclinicalimpact oftheassayonmanagementofpatientswithpharyngitis.
