Methods:SerumYKL-40levelwasdeterminedbyenzyme-linkedimmunosorbentassay.Overallsurvival(OS)wasevaluated withtheKaplan-Meiermethodandcomparedbythelog-ranktest.MultivariatestudywithCoxproportionalhazardmodel wasusedtoevaluateindependentprognosticvariablesofOS. Results:ThemedianpretreatmentserumYKL-40inHCCpatientswithwassignificantlyhigherthanthatinhealthycontrols (P,0.001). The YKL-40 could predict survival precisely either in a dichotomized or continuous fashion (P,0.001 and P=0.001, respectively). MultivariateCoxregression analysisindicated that serum YKL-40was anindependentprognostic factorforOSinHCCpatients(P=0.001).Infurtherstratifiedanalyses,YKL-40coulddiscriminatetheoutcomesofpatients withlowandhighalpha-fetoprotein(AFP)level(P=0.006and0.016,respectively).Furthermore,thecombinationofserum YKL-40andAFPhadmorecapacitytopredictpatients’outcomes.
