Abstract Introduction:Thetriggeringreceptorexpressedonmyeloidcells-1(TREM-1)isknowntobeexpressedduring bacterialinfections.WeinvestigatedwhetherTREM-1isalsoexpressedinnon-infectiousinflammationfollowing traumaticlungcontusion. Methods:Inastudypopulationof45adultpatientswithmultipletraumaandlungcontusion,weobtained bronchoalveolarlavage(BAL)(blindsuctioningof20mlNaCl(0.9%)viajetcatheter)andcollectedbloodsamples attwotimepoints(16hoursand40hours)aftertrauma.Posthocpatientswereassignedtooneoffourgroups radiologicallyclassifiedaccordingtotheseverityoflungcontusionbasedontheinitialchesttomography. ConcentrationofsolubleTREM-1(sTREM-1)andbacterialgrowthweredeterminedintheBAL.sTREM-1,IL-6,IL-10, lipopolysaccharidebindingprotein,procalcitonin,C-reactiveproteinandleukocytecountwereassessedinblood samples.PulmonaryfunctionwasevaluatedbythepaO2/FiO2ratio. Results:ThreepatientswereexcludedduetopositivebacterialgrowthintheinitialBAL.In42patientstheseverity oflungcontusioncorrelatedwiththelevelsofsTREM-116hoursand40hoursaftertrauma.sTREM-1levelswere significantly(P0.01)elevatedinpatientswithseverecontusion(2,184pg/ml(620to4,000pg/ml))incomparison withpatientswithmild(339pg/ml(135to731pg/ml))orno(217pg/ml(97to701pg/ml))contusion40hours followingtrauma.AtbothtimepointsthepaO2/FiO2ratiocorrelatednegativelywithsTREM-1levels(Spearman correlationcoefficient=-0.446,P0.01). Conclusions:sTREM-1levelsareelevatedintheBALofpatientsfollowingpulmonarycontusion.Furthermore,the levelsofsTREM-1intheBALcorrelatewellwithboththeseverityofradiologicalpulmonarytissuedamageand functionalimpairmentofgasexchange(paO2/FiO2ratio).
