Focalboneerosionisduetoexcessboneresorptionofosteoclasts.Tumornecrosisfactorreceptor-associated factor6(TRAF6)isoneofthecriticalmediatorsbothininflammatorysignalpathwayanddifferentiationand resorptionactivityofosteoclasts.HereweaimedtoinvestigateTRAF6expressioninRAsynoviumandits correlationwithhistologicalsynovitisseverityandradiologicaljointdestructioninRA. Methods:Synovitisscorewasdeterminedinneedlebiopsiedsynoviumfrom44patientswithactiveRA.Synovium fromninepatientswithosteoarthritis(OA)andsevenwithorthopedicarthropathies(Orth.A)wereenrolledas“less inflamed”diseasecontrols.SerialsectionswerestainedimmunohistochemicallyforTRAF6aswellasCD68 (macrophage),CD3(Tcell),CD20(Bcell),CD38(plasmocyte),CD79a(Blineagecellsfrompre-Bcelltoplasmocyte stage),andCD34(endothelialcell).DoubleimmunofluorescencestainingofTRAF6andCD68weretested.Densities ofpositivestainingcellsweredeterminedandcorrelatedwithhistologicaldiseaseactivity(synovitisscore)and radiographicjointdestruction(Sharpscore). Results:TRAF6expressionwasfoundintheintimalandsubintimalareaofRAsynovium,withintensestaining foundintheendochylemaandnucleusofintimalsynoviocytesandsubintimalinflammatorycells.Double immunofluorescencestainingshowedTRAF6wasexpressedinmostoftheintimalcellsandobviouslyexpressedin CD68+cellsandsomeotherCD68-cellsinsubintimalarea.SynovialTRAF6wassignificantlyover-expressedinthe RAgroupcomparedwiththeOAandOrth.Agroup(2.53±0.94vs.0.72±0.44and0.71±0.49,P0.0001).
