Methods:Sidestreamcapnographywasperformedduringmechanicalventilationinpatientsundergoingelective cardiacsurgery(n=144).Theairwayresistance(Raw),totalrespiratoryresistanceandCrsdisplayedbythe ventilator,thepartialpressureofarterialoxygen(PaO2)andSIII weremeasuredintimedomain(ST-III)andina smallercohort(n=68)byvolumetry(SV-III)withandwithoutnormalizationtotheaverageCO2phaseIII concentration.Measurementswereperformedatpositiveend-expiratorypressure(PEEP)levelsof3,6and9 cmH2Oinpatientswithhealthylungs(GroupHL),andinpatientswithrespiratorysymptomsinvolvinglow(Group LC),medium(GroupMC)orhighCrs(GroupHC). Results:ST-III andSV-III exhibitedsimilarPEEPdependenciesanddistributionbetweentheprotocolgroupsformed onthebasisofCrs.AwideinterindividualscatterwasobservedintheoverallRaw-ST-III relationship,whichwas primarilyaffectedbyCrs.DecreasesinRawwithincreasingPEEPwerereflectedinsharpfallsinSIII inGroupHC, andinmoderatedecreasesinSIII inGroupMC,whereasST-III wasinsensitivetochangesinairwaycaliberinGroups LCandHL. Conclusions:SIII assessedinthetimedomainandbyvolumetryprovidemeaningfulinformationaboutalterations inairwaycaliber,butonlywithinanindividualpatient.AlthoughST-III maybeofvalueforbedsidemonitoringof theairwayproperties,itssensitivitydependsonCrs.Thus,assessmentofthecapnogramshapeshouldalwaysbe coupledwith
