Methods:ThestudywasapprovedbytheInstitutionalReviewBoardofourhospital. Atotalof276women(meanage=50.8years,SD11.8)withsuspiciousfindingson mammogramsorultrasoundreceivedIRimagingofthebreastbeforeexcisional biopsy.Theinterpretingradiologistsscoredthelesionsusingascoringsystemthat combinesfiveIRsigns.TheROC(receiveroperatingcharacteristic)curveandAUC (areaundertheROCcurve)wereanalyzedbytheunivariatelogisticregressionmodel foreachIRsignandanage-adjustedmultivariatelogisticregressionmodelincluding 5IRsigns.Thecut-offvaluesandcorrespondingsensitivity,specificity,Youden’ sIndex (Index=sensitivity+specificity-1),positivepredictivevalue(PPV),negativepredictive value(NPV)wereestimatedfromtheage-adjustedmultivariatemodel.Themost optimalcut-offvaluewasdeterminedbytheonewithhighestYouden’ sIndex. Results:Fortheunivariatemodel,theAUCoftheROCcurvefromfiveIRsigns rangedfrom0.557to0.701,andtheAUCoftheROCfromtheage-adjusted multivariatemodelwas0.828.FromtheROCderivedfromthemultivariatemodel, thesensitivityofthemostoptimalcut-offvaluewouldbe72.4%withthe correspondingspecificity76.6%(Youden’sIndex=0.49),PPV81.3%andNPV66.4%. Conclusions:Weestablishedaninterpretiveage-adjustedmultivariatemodelforIR imagingofthebreast.Thecut-offvaluesandthecorrespondingsensitivityand specificitycanbeinferredfromthemodelinasubpopulationfordiagnosticpurpose.
