慢性肾功能不全患者血管活性物质测定在血液净化中的临床研究 作者：樊雨良，王美雯，陆遥，张凯璇，刘沁，谢有娣，金蕾 【摘要】 目的 评价血管紧张素、氨甲酰血红蛋白、醛固酮及甲状旁腺激素等在血液净化充分性中的意义。 方法 120例慢性肾功能衰竭患者，按采用不同的透析膜分为3组：即聚砜膜、醋酸膜、铜仿膜。用放射免疫法测定其血管紧张素(AngⅡ)及醛固酮(ALD)浓度，化学发光法测甲状旁腺激素(PTH)。采用不同的血流量200ml/min及250ml/min比较。结果 对体内中、小分子物质清除率，聚砜膜＞醋酸膜＞铜仿膜。结论 高血流量、高通透性人工合成聚砜膜，对 治疗 慢性肾功能衰竭，提高肾功能不全患者的身心健康及生活质量、延长患者生命有很重要的临床意义。 【关键词】 慢性肾功能衰竭；血管紧张素；醛固酮；甲状旁腺激素；血液透析 【Abstract】 Objective Appraisal the significance of Ang，CarHb，ALD and PTH fix on clinical research of blood purification to reach the sufficient result.Methods 120 patients with chronic renal failure，according to adopt different dialysis membrane divide into three groups：namely polysulfone membrane，acetic acid membrane，cuprum imitate membrane.With immunoradiometric assays to determine thEir angiotensin(Ang Ⅱ)and aldosterone(ALD) density，chemistry iuminescence law，examine parathyroid hormone(PTH).Adopt different blood flow 200ml/min and 250ml/min compare.Results The function on body by analysis the clearance rate of small nember material，polysulfone membrane＞acetic acid membrane＞the cuprum imitates the membrane.Conclusion High blood flow，high permeability of artificial synthesis polysulfone membrane，treat chronic renal failure，improve kidney function on the whole physical and mental health and life quality of patient，lengthen patient life and the important clinical meaning. 【Key words】 chronic renal failure；Angiotensin ；ALD；PTH；hemodialysis 慢性肾功能衰竭时肾脏严重受损，球旁装置对血容量调节肾素分泌的生理功能受到破坏，引起血容量和肾素两者的平衡失调。本文自2002年1月～2005年5月，收治120例慢性肾功能衰竭患者 应用 3种不同的中空纤维血透器聚砜膜、醋酸膜、铜仿膜血液净化。通过测定血浆Ang、ALD、PTH及其它生化指标参数变化， 研究 透析的充分性，现报道如下。 1 资料与方法 1.1 材料 主要仪器设备：血透机使用瑞典GAMBRO公司AK-200、AK-95及美国Baxtr公司550血透机。透析器为德国FRESENTUS F-6聚砜膜1.3中空纤维透析器、日本Nipro FB-130A双醋酸膜中空纤维透析器、国产山西华鼎-120铜仿膜中空纤维透析器。透析模式均为碳酸盐透析，透析液流速500ml/min、透析时间均为5h。 1.2 临床资料 1.2.1 病例选择标准 原发病经各项物理及实验室辅助检查，按全国有关诊断标准确诊、各种病因导致终末期尿毒症血液透析患者。各种病因导致慢性肾衰患者120例，男79例，女41例，年龄45～80岁，平均62岁。其中慢性肾炎50例，糖尿病肾病21例，高血压肾动脉硬化13例，慢性肾盂肾炎12例，梗阻性肾病7例，晚期肿瘤5例，移植肾失败4例，狼疮性肾炎3例，先天性多囊肾2例，肾动脉狭窄2例，淀
