老年人上消化道出血114例临床分析 【摘要】 目的 探讨老年人上消化道出血的临床特点。 方法 回顾性 分析 我院1997年1月～2006年12月收治的老年人上消化道出血114例的临床特点，并与同期中青年病例比较。结果 老年人上消化道出血仍以消化性溃疡多见（43.5%），但胃溃疡出血发生率增加（22.58%），稍高于十二指肠溃疡发病率（20.97%），与中青年人以十二指肠溃疡为主有显著差异；胃癌出血发生率显著增加（18.28%），占病因第二位，与中青年人相比差异显著；老年患者合并症、并发症、再出血率和死亡率显著高于中青年组。结论 老年人上消化道出血仍以消化性溃疡为多见，胃溃疡、胃癌出血发生率增加，并发症多，再出血率和死亡率高。 【关键词】 上消化道出血；老年人；病因 Clinical characters of 114 aged patients with upper gastrointestinal hemorrhage CHEN WEi-hong.General Department，Xincheng Hospital of Jiangmen，Jiangmen 529100，China ［Abstract］ Objective To investigate the clinical characters of upper gastrointestinal hemorrhage in aged patients.Methods We retrospectively analyzed the clinical features of 186 aged patients with upper gastrointestinal hemorrhage treated in our hospital from Jan.1997 to Dec.2006，and compared them with that of 418 young cases during the same time.Results The main cause of upper gastrointestinal hemorrhage in aged patients was also peptic ulcer （43.5%），but the bleeding rate of gastric ulcer（22.58%）was more than that of duodenal ulcer （20.97%），rather than that duodenal ulcer bleeding was the main cause in young patients.Gastric cancer bleeding had significantly increased and became the second main cause.To compare with the young，the aged had more morbidities，more complications，more rehemorrhages and mortalities.Conclusion In aged patients，peptic ulcer is also the main cause of upper gastrointestinal hemorrhage，but gastric ulcer and cancer bleeding had increased.They had more complications，rehemorrhages and mortalities. ［Key words］ upper gastrointestinal hemorrhage；the aged；etiology 上消化道出血是消化内科常见的急症，尤其是老年人的上消化道大出血，出血较急，临床表现多不典型，伴随疾病多，病死率较高。因此提高对老年人急性上消化道出血病因的认识，有助于提高本病的诊断率及治愈率。现回顾性分析我院1997年1月～2006年12月间收治的老年人上消化道出血114例的临床资料，并与同期收治的中青年病例256例进行比较，分析报告如下。 1 临床资料 1.1 一般资料 将患者分为2组，老年组114例，男81例，女33例，男女之比2.5∶1；年龄60～89岁，平均（73±8.5）岁。对照组256例，男195例，女61例，男女之比3.21；年龄17～59岁，平均（37±7.8）岁。 1.2 临床诊断 根据呕血、便血的临床表现，大便潜血试验阳性，全部病例均经胃镜确诊，并排除下消化道出血。 1.3 统计学方法 应用 SPSS 11.0软件对数据进行统计学分析，计量资
