腰硬联合麻醉和硬膜外麻醉在早期妊娠孕妇的急性阑尾炎麻醉中的应用比较 　　[摘要] 目的 探析腰硬联合麻醉和硬膜外麻醉在早期妊娠孕妇的阑尾炎麻醉中的应用价值。方法 整群选取2015年8月―2016年3月因急性阑尾炎需急行手术的56例早期妊娠孕妇，按照麻醉方式的不同，随机分为观察组和对照组。其中观察组予以腰硬联合麻醉，共28例；对照组病例应用硬膜外麻醉，共28例。观察并比较两组患者的麻醉效果、患者与胎儿的血流动力学情况以及麻醉不良反应的发生情况。 结果 观察组患者麻醉有效率100.00%，对照组患者麻醉有效率78.57%；观察组患者发生不良反应发生率3.57%，对照组患者不良反应发生率21.43%，与采用硬膜外麻醉的对照组相比，予以腰硬联合麻醉的观察组患者麻醉总有效率更高（P0.05）。结论 罹患急性阑尾炎的早期妊娠孕妇采用腰硬联合麻醉对孕妇以及胎儿安全性较高，麻醉效果确切，临床推广价值较高 [关键词] 腰硬联合麻醉；硬膜外麻醉；早期妊娠孕妇；阑尾炎；麻醉；应用效果 [中图分类号] R5 [文献标识码] A [文章编号] 1674-0742（2016）06（a）-0097-03 [Abstract] Objective To study the value of combined spinal epidural anesthesia and epidural anesthesia in the early pregnancy of pregnant women. Methods Group selection a total of 2015August to 2016 March due to acute appendicitis should be sped surgery and treated of 56 cases of early pregnant women， according to the mode of anesthesia， were randomly divided into the observation group and the control group. Conclusion Early pregnant women suffering from appendicitis using combined spinal epidural anesthesia for pregnant women and fetal high safety， anesthetic efficacy， higher clinical value.
