Glucose intolerance is caused by an inability of the ?-cells to secrete the amount of insulin required to compensate for insulin resistance. Many individuals will stay glucose tolerant despite considerable insulin resistance as long as there is sufficient ?-cell secretory capacity to compensate. Likewise, an insulin sensitive individual can stay glucose tolerant with a reduced level of ?-cell function. ? Weyer and colleagues demonstrated this process in studies involving Pima Indians – a race of American Indians at high risk of developing diabetes.1 As long as ?-cell function compensates for insulin resistance, the individual remains glucose tolerant. However, an inability to compensate leads to the individual ‘dropping off the curve’ and impaired glucose tolerance (IGT) ensues.
