超声在胎儿消化系统异常产前诊断中应用价值 　　[摘要] 目的 探讨超声在胎儿消化系统异常（Digestive system abnormalities，DSA）产前诊断中的应用价值。 方法 选取2014年1月～2015年12月于我院接受产前超声诊断的孕妇1986例，所有孕妇均行超声探查，明确胎儿消化系统发育情况，所有疑似DSA病例均行产后随访，胎儿消化道严重畸形孕妇行引产尸检或产后复查。 结果 所有1986例胎儿中，共检出8例胎儿存在DSA，其中十二指肠狭窄或闭锁3例，肠管扩张、肠管回声增强、食管闭锁、巨结肠、胎粪性腹膜炎各1例，DSA胎儿检出率为0.40%，漏诊2例，漏诊率为20.00%。8例中误诊1例，误诊率为12.50%，正常分娩5例（62.50%），引产1例（12.50%），胎死宫中1例（12.50%）。 结论 超声产前诊断胎儿DSA准确率高，且无创、无痛，具有较高的应用价值 [关键词] 超声；胎儿；消化系统异常；产前诊断 [中图分类号] R445.1；R714.53 [文献标识码] B [文章编号] 1673-9701（2016）36-0071-03 [Abstract] Objective To explore the value of ultrasound in prenatal diagnosis of digestive system abnormalities（DSA） in the fetus. Methods 1986 cases of pregnant women who received prenatal ultrasound diagnosis in our hospital from January 2014 to December 2015 were selected， and all pregnant women underwent ultrasonography to confirm the fetal digestive system development. All cases suspected of DSA were followed up after birth， and the pregnant women with severe malformations of the digestive tract of fetus were treated with induction of labor and autopsy of the fetus or postpartum review of post-mortem. Results In all 1986 fetuses， 8 cases of fetal DSA were detected， including duodenal stenosis or atresia in 3 cases， and intestinal dilatation， intestinal echo enhancement， esophageal atresia， intestinal atresia， megacolon， meconium peritonitis in 1 case， respectively. fetal DSA detection rate was 0.40%； and there were 2 cases of missed diagnosis， with missed diagnosis rate 20.00%； In the 8 cases， there was 1 case of misdiagnosis， with misdiagnosis rate 12.50%； and there were 5 cases of normal delivery（62.50%）， 1 case of abortion（12.50%） and 1 case of fetal death in utero. Conclusion Ultrasound prenatal diagnosis of fetal DSA has high accuracy， and is noninvasive and painless， with a high value.
