[Abstract] Objective Toinvestigatethereasonsofrecrrentconstipationafterradicalmac-
rosigmoidoperation.Methods 392 casesofhirschsprngsdisease(HD)sbjecttosrgeryinorhos-
pitalfrom1995 to2005 werereviewed, inwhich32 patientswerereoperatedforrecrrentconstipation.The
srgicalmethod,spathologicalfinding,sincidenceofenterocolitisanddefecationofrecrrentcaseswerean-
alyzed.Reslts TherecrrentrateofsrgicaltreamtentforHDinorhospitalwas4.5%.Therecrrent
rateofsigmoidordescendingcolonresection(16.0%, 26 /162 )wassignificantlyhigherthanthatofcolon
sbradicalresection(2.1%, 4 /189 )(P0.01).Among32 recrrentpatient,stherewere26 casesof
hirschsprngsallieddisease(HAD)orHDassociatedHAD.Thereweremanyreasonsforrecrrenc,ein-
cldingnon-completedresectionofHADproximalintestineorsecondaryHADpathologicalchang,enon
-completedresectionofHDintestine;notenoghresectionofthedistalrectm, anastomoticstenosi,sanal
strictreorachalasia.Theincidenceofenterocolitisinrecrrentpatientswassignificantlyhigherthanthat
inthosewithotrecrrence(P0.05 ).Conclsion HADpatientsaremoressceptibletorecrcon-
stipationthanHD.Theconstipationrecrrenceratecandeclineifthefollowingmeasresaretaken:com-
pletelyresectingtheproxmialanddistalpathologicalbowelandmakingtheanastomosisaslowaspossible;
