94例急性肾功能衰竭临床研究【摘要】 目的 总结急性肾功能衰竭的病因、临床特点。方法 收集2000年1月至2008年12月临床资料完整的急性肾功能衰竭患者94例,按年龄分为老年组(61岁)和青年组(≤60岁),分析两组患者的病因、临床特点等。结果 老年组患者为35例,青年组59例;老年组与青年组病因构成不同,老年组肾前性和肾后性比例高于青年组,而肾性比例低于青年组;老年组的主要病因为感染和恶性肿瘤,青年组为药物和血容量不足;与青年组相比,老年组合并高血压、冠心病、糖尿病等慢性基础疾病比例高;老年组治疗效果较差。结论 老年急性肾功能衰竭患者与青年患者病因构成不同,老年患者合并慢性基础疾病的比例高、治疗效果差。 【关键词】 急性肾功能衰竭;病因;合并症 Clinical study of 94 cases with acute renal failureWANG Jie.The First Hospital of Quanzhou City,Fujian Province 362000,China 【Abstract】 Objective To analyze the etiologies and clinical characteristics of 94 cases with acute renal failure(ARF).Methods 94 Patients with ARF in our hospital from January 2000 to December 2008 were divided into two groups:elderly patients(61 years old)and young patients(≤60 years old).All the patients were retrospectively investigated for their etiology and clinical characteristics.Results The elderly group had 35 patients and the young group had 59 patients.The causes of the two groups were different significantly,elderly patients with pre-renal reasons and post-renal reasons were higher than those in the young,and the causes of nephritis in the elderly was lower.The main causes in the elderly were infection and m alignant tumor.The main causes in young patients were drugs and volume depletion.Previous chronic disease was more prevalent in the elderly,such as hypertension,coronary heart disease and diabetes.Therapeutic effect was worse in the elderly.Conclusion To compare the acute renal failure(ARF)in the elderly and young patients.The causes of the two groups were different significantly.Previous chronic diseases were more prevalent in the elderly compared to the young and therapeutic effect was worse in the elderly. 【Key words】 Acute renal failure; Cause; Complication 急性肾功能衰竭(ARF)是临床上常见的危重急症。据文献报道,ARF病死率约为10%~15%,同时合并多器官功能衰竭的ARF患者,病死率更高达50%~90%不等[1]。本文回顾性分析2000年1月至2008年12月泉州市第一医院94例ARF患者的临床资料,着重探讨患者的病因、临床特点等,旨在为ARF患者的诊断和治疗提供依据,并达到重视预防的目的。 1 对象与方法 1.1 ARF的诊断标准 肾功能短期内(数小时至数周)急剧下降,肾小球滤过功能
