依立替康联合顺铂治疗广泛期小细胞肺癌临床观察【摘要】 目的 观察伊立替康联合顺铂(IP)治疗广泛期小细胞肺癌(SCLC)的近期疗效和毒不良反应。方法 60例广泛期小细胞肺癌患者，随机分为IP组30例和EP组(依托泊苷+顺铂) 30例，两组病例的临床资料具有可比性。疗效评价：IP组CPT-11 60 mg/m2静脉滴注90 min,第1、8、15天; DDP 25 mg/m2第1、8、15天。EP组依托泊苷(VP-16) 100 mg/m2,1~5 d; DDP 80 mg/m2 1~3 d,均静脉滴注, 28 d为1个周期;至少治疗2个周期。按标准评价疗效和毒副作用。结果 入组60例患者中,均可评价疗效及不良反应, IP组总有效率80．0%;EP组总有效率56．7%。IP组在近期疗效、血液学毒性方面都优于EP组，经统计学处理两组之间差异有统计学意义。结论 CPT-11联合顺铂方案治疗广泛期小细胞肺癌有较好的临床疗效,耐受性好。 【关键词】 依立替康;顺铂;小细胞肺癌;化学治疗 Clinical observation of irinotecan combining cisplatin on extensive small- cell lung cancer WU Hui-juan,ZHAO Yan-qiu,LIUTao.Department of Internal Medicine, Henan Tumor Hospital,Henan Province 450008,China 【Abstract】 Objective To observe the clinical efficacy and side reactions of irinotecan (CPT-11) combining cisplatin for extensive small-cell lung cancer (SCLC). Methods 60 cases with SCLC were randomly divided into two groups(IP and EP groups).The IP group of 30 patientswere treated with a combination of irinotecan and cisplatin.The EP group of 30 patientswere treated with a combination of Etoposid and cisplatin. Clinical data of two groups had comparability.Evaluation of curative effect: The IP group: irinotecan 60 mg/m2,iv the first,eighth,15th day; cisplatin 25 mg/m2,iv the first,eighth,15th day; The EP group: Etoposid 100 mg/m2iv 1~5 days; Cisplatin 80 mg/m2,iv 1~3 days．Every four weeks constituted one treatment cycle, two cycles constituted one course of treatment. Results Of the 60 evaluable patients,there were 2 complete responses and twenty-two partial responses(response rate 80．0%)in the IP group,and there were three complete responses and fourteen partial responses(response rate 56．7%) in the EP group. Of 2-group patients, the IP group surpassed EP group in recent curative effect and toxic adverse reactions. There was a great difference between two groups by statistical analysis.Conclusion The regimen of irinotecan combining cisplatin on extensive SCLC has good clinical efficacy and tolerability. 【Ke
