某院2010年腹股沟疝气手术预防性使用抗菌药物调查及研究【摘要】目的 调查和分析医院疝气手术围手术期预防性用药的合理性。方法 回顾性调查该院2010年疝气手术病历120例，对资料进行统计分析。结果 120例Ⅰ类切口手术患者均使用了抗菌药物预防感染，使用率为100%；其中一联用药占95.0%,二联用药占5.0%；预防用药共6种，头孢菌素类占95.0%。手术预防用药时间统计为0～7 d，其中预防性用药时间≥2 d的占42.5%。结论 该院Ⅰ类切口手术围手术期预防使用抗菌药物存在选药起点较高，种类欠妥，疗程较长等问题，亟需加强临床医生规范化用药管理。 【关键词】腹股沟疝 围手术期 抗菌药物 中图分类号：R978.5文献标识码：A文章编号：1005-0515（2011）3-032-03 Analysis of the usage of prophylactic antibiotics in herniorrhaphy Xu Chunhong Quan Li Liu Yan Cao Ting （Department of Pharmacy, the Peoples Hospital of Shifang, Shifang Sichuan 618400, China） 【Abstract】Objective To investigate and analyze the rationality ofthe usage of prophylactic antibiotics in herniorrhaphy during perioperative period in hospital. Methods The 120 case files were investigated and analyzed, which were selected from the hospital during 2010. Results The prophylactic antimicrobial usage rate was 100% in the 120 patients; There were 95.0% for one drug use and 5.0% for two antibacterials use. A total of antibiotics are six and the most frequently used antimicrobial agents was cephalosporins(95.0%). The time of prevention usage was(0~7) d, 42.5% used more 2d. Conclusion There are many problems of antibiotics using on type 1 of operative incision during perioperative period, the prophylactic medication was in high level from beginning, the category of antibiotics was improper , and the course of treatment was too long. It is urgent to strengthen the normative management on clinicians. 【Key words】herniorrhaphy perioperative period antibiotics 外科围手术期预防使用抗菌药物是预防手术后感染的有效措施[1]，但目前围手术期抗菌药物滥用的现象非常普遍[2]，为规范医院抗菌药物的临床应用，笔者对某医院120例腹股沟疝气清洁手术预防性使用抗菌药物进行了汇总分析，旨在为临床合理使用抗菌药物提供参考和依据。 1 资料与方法 1.1 一般资料 随机抽取某院2010年腹股沟疝气手术病历120例。 1.2 方法 按自制统计表格：姓名、性别、年龄、诊断、手术部位、手术时间、抗菌药物的名称、用法用量、用药的起止时间等，进行统计分析。 1.3 评价标准 参照卫生部《抗菌药物临床应用指导原则》、外科《应用抗菌药物治疗外科感染的指导意见》的要求，以及“医院管理年”评审标准，进行统计评价。Ⅰ类切口手术一般不预防使用抗菌药物，若清洁手术的手术范围大、手术时间长、手术涉及重要器官、异物植入手术、高龄或免疫缺陷者等情况时，可预防使用抗菌药物，但需严格掌握用药适应证、药物选择、给药起始与持续时间、给药方法要按照《抗菌药物临床应用指导原则》有关规定，术前0.5~
