腹腔镜下套扎法治疗输卵管间质部妊娠11例临床观察[摘要] 目的：探讨腹腔镜下套扎法治疗输卵管间质部妊娠临床效果。方法：回顾性分析2006年1月～2007年12月腹腔镜下套扎法治疗治疗的11例输卵管妊娠的临床资料，所有患者在静脉全麻醉下行腹腔镜手术。清除输卵管间质部妊娠组织，在包块与子宫交界处用双极电凝刀沿包块靠输卵管侧缓慢凝切一槽沟，1.0?乔线体外自制线圈套结置入腹腔套扎输卵管间质部。双极电凝并剪开输卵管间质部浆肌层，取出妊娠组织再收紧套扎线。结果：腹腔镜治疗均获成功，无持续性异位妊娠发生。术中出血量、住院天数及日常生活恢复时间理想。结论：腹腔镜下自制沟槽套扎法治疗输卵管间质部妊娠是一种安全、疗效确切的手术方法，具有术中出血少、住院时间短、术后恢复快的优势，为首选治疗方法。 [关键词] 腹腔镜；套扎；输卵管间质部妊娠 [中图分类号] R714 [文献标识码]A [文章编号]1673-7210（2008）06（c）-052-02 Clinical observation of laparoscopic interstitial tubal pregnancy therapy with loop ligature method in 11 cases ZHOU Feng-zhen,ZHOU Xia-ping (Department of Gynecology the Sencond People’s Hospital of Shenzhen,Affiliated to Nanfang Medical University,Shenzhen518029,China) [Abstract] Objective:To approach clinical effect of laparoscopic interstitial tubal pregnancy therapy with loop ligature method.Methods: Retrospective analyzed clinical data of laparoscopic interstitial tubal pregnancy therapy with loop ligature method in 11 cases from January 2006 to December 2007, under persistence epidural anesthesia and intravenous anesthesia combination, all patients were done laparoscopic surgery. Cut tubal isthmus and mesenterium to womb which was inferior enclosed mass, used ultrasound knife to cut a slot slowly around enclosed mass near womb side 3 mm, in enclosed mass and womb juncture, put self-made winding loop into abdominal cavity. Used ultrasound knife to cut enclosed mass off, in enclosed mass and womb juncture, above loop ligature line 3 mm.Results: Therapeutic laparoscopy were all successful, didn?t occur persistence ectopic gestation. Bleeding volume in surgery, hospital days and recovery time of daily life were all good.Conclusion: Laparoscopic interstitial tubal pregnancy therapy with loop ligature method is a safe and curative effect exact operation method, its advantage was insufficiency of bleeding in surgery, short hospital days and rapid recovery time, it is a preferred treatment.
