螺内酯对急性心肌梗死患者心功能及P波离散度影响【摘要】目的：观察螺内酯对急性心肌梗死患者P波离散度及心功能影响。方法：将发病在48小时内的急性前壁心肌梗死患者，随机分为治疗组和对照组，对照组给抗凝、抗血小板聚集、调脂治疗，根据病情给适量的利尿剂，硝酸酯类、血管紧张素转换酶抑制剂、β受体阻滞剂只要无禁忌证都足量应用。治疗组除上述治疗外给螺内酯25 mg，每天1次，共2周。结果：治疗组房性心律失常发生率明显低于对照组，P波离散度明显短于对照组，左室射血分数高于对照组（P0.05）。结论：急性心肌梗死患者早期应用螺内酯能稳定电生理，改善P波离散度，改善心功能。 【关键词】螺内酯；急性心肌梗死；心功能；P波离散度 文章编号：1009-5519（2007）16-2385-02 中图分类号：R5 文献标识码：A Effect of spironolactone on the heart function and P wave dispersion degree in patients with acute myocardial infarction WANG Feng-lan,WANG Qiu-fen,MA Li-ping (Department of Cardiology,The People’s Hospital of Puyang,Henan 457000,China) 【Abstract】Objective：To observe the effect of spironolactone on P wave dispersion degree and the heart function in patients with acute myocardial infarction.Methods：The patients suffering from acute myocardial infarction within 48 hours were divided into the treatment group and control group at random.The anticoagulant,antiplatelet aggregation and regulatory-lipid treatment was used in both groups,the adequate diuretics,nitrate ester and angiotensin converting enzyme inhibitor were used according to the patient’s condition,and the sufficient β-receptor blocker was given provided that these patients had no contraindication.Besides those,the treatment group was added with spironolactone 25 mg once daily for 2 weeks.Results：The incidence of atrial arrhythmic in the treatment group was significantly lower than that in the control group,the dispersion degree of P wave in the treatment group was significantly shorter than that in the control group,while the left ventricle ejection fraction was higher than that in the control group(P0.05).Conclusion：Using spironolactone in treating acute myocardial infarction early can stabilize the electrophysiology of myocardial cell and improve the heart function and P wave dispersion degree. 【Key words】Spironolactone；Acute myocardial infarction；Heart function；P wave dispersion degree P波离散度（PD）是反映心房肌传导不均一性和电活动不稳定的一项指标，急性心肌梗死患者的心功能及房性
