重症患者早期肠内营养支持治疗临床探究[摘要] 目的 探讨重症患者早期肠内营养支持治疗效果。方法 100例重症患者按给养方式分为肠外营养组40例（PN）和肠内营养组60例（EEN），入ICU 48h内分别进行肠外营养和肠内营养，EEN组分为达标组35例（A组）和未达标组25例（B组）。结果 治疗后EEN组和PN组血浆中TP、Alb、Hb均显著升高（P＜0.05），EEN组与PN组及A组与B组间有显著差异（P＜0.05）。PN组ALT、Cr较EEN组显著升高，EEN组CRP下降较PN组显著（P＜0.05）。A组与B组ALT、Cr、CRP比较有显著差异（P＜0.05）。结论 早期达标的肠内营养能显著改善患者营养状况，保护消化道黏膜防止并发症的发生。 [关键词] 肠内营养；肠外营养；危重症 [中图分类号] R459.3 [文献标识码] B [文章编号] 1673-9701（2011）36-49-02 Clinical Study on the Effect of Early Enteral Nutrition in Critically Ill Patients DING Peizhi Department of ICU，Central Hospital of Lishui in Zhejiang Province，Lishui 323000，China [Abstract] Objective To investigate the effect of early enteral nutrition（EEN） in critically ill patients. Methods All 100 cases of critically ill patients were divided into parenteral nutrition（PN） group of 40 cases and EEN group of 60 cases，they were given respectively PN and EEN within 48h when in the ICU. EEN group was divided into the up to scratch group of 35 cases（group A）and not-up to scratch group of 25cases（group B）. Results After treatment，TP，Alb and Hb of PN and EEN group significantly increased（P＜0.01），and there were significant differences of them between PN and EEN group as well as A and B group（P＜0.05）. The ALT and Cr of PN increased higher than EEN group（P＜0.05），and the CRP of EEN group decreased lower than PN group（P＜0.05）.There were significant differences of ALT，CrA and CRP between A and B group（P＜0.05）. Conclusion The EEN up to scratch can significantly improve the nutritional status of critically ill patients，protect the gastrointestinal mucosa， prevent complications. [Key words] Enteric nutrition；Parenteral nutrition；Critically ill 重症患者呈现明显的负氮平衡、低蛋白血症等严重营养不良的状态，因此营养支持治疗是重症疾病治疗的重要组成部分。应激反应是导致重症患者肠道黏膜损伤、全身炎症反应等继发损伤的重要因素之一。在重症早期（48h内）给予营养治疗对维护机体代谢及组织、器官的功能，减轻继发性损伤有重要的临床意义。随着营养技术的发展，早期肠内营养在各类重症患者中被采用。本研究观察早期肠内营养与肠外营养对重症患者营养状态的改善及保护肠道黏膜的效果，现报道如下。 1 资料与方法 1.1 一般资料 选择2008年7月～2011年7月我院ICU住院重症患者100例，男62例，女38例，年龄20～78岁，平均年龄（61.78±9.56）岁，按营养治疗方式分为肠外营
