· 144· 安 徽 医 药 Anhui Medical and Pharmaceutical Journal 2008 Feb；12(2) 住院婴幼儿肺炎支原体呼吸道感染的临床分析 肖政祥，苏 红，胡晓波 (广东省深圳市龙岗区妇幼保健院儿科，广东深圳 518172) 摘要：目的 探讨住院婴幼儿肺炎支原体(MP)呼吸道感染的临床特点和血常规改变。方法 从我院1362例患呼吸道感染的 住院儿童中选择经ELISA classicMP—IgG／IgM检测及咽拭子标本MP选择培养确诊的MP呼吸道感染住院婴儿75例、幼儿116 例为研究对象，47例确诊为MP呼吸道感染的3岁以上住院儿童为对照，分析各年龄组儿童MP呼吸道感染的发病率、病种分 布、临床特点及血WBC和PLT变化。结果 婴儿、幼儿、3岁以上儿童MP感染分别占呼吸道感染总数的9．0％、27．1％、27． 2％，上、下呼吸道感染比例分别为1：18、1：12及1：2．6；婴儿MP感染后咳嗽1周以上伴痰鸣及哮鸣音较幼儿和3岁以上儿童 明显增多；MP感染后95％以上婴幼儿血WBC正常或增高，血PLT增高者也达50％以上。结论 MP已是我院婴幼儿呼吸道感 染的重要病原，年龄越小下呼吸道感染越多，较长咳嗽、痰鸣音是婴儿MP感染的主要特点。 关键词：肺炎支原体；呼吸道感染；临床特点；婴幼儿 CHnical analyses of respiratory infection by mycoplasma pneumoniae in inpatients under 3．．year．．old children XIAO Zheng-xiang，SU hong，HU Xiao-bo (Maternal and Children Healthcare Hospital ofLonggang District，Shenzhen 518172，China) Abstract：Aim To explore the clinical findings and blood routine of respiratory infection by mycoplasma pneumoniae(MP)in hospital— ized children under 3 years old．Methods 1 362 samples Nasopharyngeal aspirates for MP—selected culture or Sera for MP—IgM were col— lected．238 positive cases were divided into three groups：0～1 year—old group(n=75)，1—3 year—old group(n=1 16)，over 3 year-old group(n=47) Their incidence，clinical features，sort of respiratory infection and change of WBC／PLT were analyzed．Results The MP incidence of respiratory infectious diseases in three groups were 9．0％ ，27．1％ ，and 27．2％ ，respectively．The rates of MP infection in upper respiratory tract and lower respiratory tract in three groups were 1：18，1：12，and 1：2．6，respectively．In 0—1 year-old group，the degree of cough prolonged over 1 week accompanied with rales，rhonchi，and expiratory wheezing were significantly more serious than that in 1—3 year-old or over 3 year—old group(P0．05)．Periphe
