针刺对2型糖尿病患者血清脂联素影响摘 要：目的：探讨脂联素是否参与了针刺对2型糖尿病患者的降血糖作用。方法：将80例2型糖尿病患者随机分为针刺组（40例）和优降糖组（40例），针刺组根据中医理论辨证施治12周，优降糖组则根据血糖水平调节用药剂量12周，检测治疗前后空腹血糖、空腹胰岛素、血清脂联素，计算胰岛素敏感指数和胰岛素抵抗指数。结果：治疗后，针刺组和优降糖组的空腹血糖、胰岛素抵抗指数均明显下降，胰岛素敏感指数明显回升；针刺组空腹胰岛素下降明显，脂联素水平明显回升，优降糖组空腹胰岛素有所上升，脂联素水平无明显变化。结论：针刺对脂联素的调节可能是针刺降血糖、增加胰岛素敏感性、改善胰岛素抵抗的机制之一，优降糖具有良好的降糖效果但对脂联素无影响。 关键词：针刺；2型糖尿病；脂联素；胰岛素抵抗 中图分类号：R259.871 文献标识码：A 文章编号：1673-7717（2011）04-0715-03 The Effect of Acupuncture on Serum Adiponectin in Patient with Type 2 Diabetes Mellitus ZHANG Qun-yan, ZHAO Zhi-ming, ZHAO Ling-jie, CAI Hui （Department of Integrated Traditional Chinese and Western Medicine, Nanjing General Hospital of Nanjing Military Command, PLA, Nanjing 210002, Jiangsu,China） Abstract:Objective: To study the relationship between adiponectin and type 2 diabetes mellitus, as well as the effect of acupuncture on serum level of adiponectin in patients with type 2 diabetes. Methods: 80 cases were devided randomly into acupunture（AG） and Glibenclamide group（GG）. AG was treated with acupuncture according to pattern of syndrome, while GG dose was regulated according to blood sugar. Fasting plasma glucose （FPG）, fasting insulin（FINS） and adiponectin （APN） were detected before and after 12 weeks treatment, at the same time insulin sensitivity index （ISI） and homeostasis model assessment-insulin resistance （HOMA-IR） were calculated. Results: After treatment for 12 weeks, FPG and HOMA-IR decreased significantly, ISI increased significantly in both AG and GG, FINS decreased while APN increased markedly only in AG. FINS increased to some extent, while change of APN was not found in GG. Conclusion: Regulation of acupuncture on adiponectin may be one of mechanisms that acupuncture lowering blood sugar, promoting insulin sensetivity and improving insulin resistance of patients with type 2 diabentes mellitus. Glibenclamide has favourable effect on lowering blood sugar, but doesn’t influence serum level of adiponectin. Key words:acupuncture
