临床输血护理规范化管理及持续改进 　【摘要】 目的：分析输血病历中护理质量管理中存在的问题，探讨通过加强护理质量管理，促进输血护理规范化，提高临床输血安全，避免医疗纠纷。方法：随机抽查笔者所在医院2012-2014年输血归档病历，每月20份，共720份。检查输血申请单、输血标本接受登记、输血（配血）报告单、输血医嘱执行、输血不良反应、输血过程护理记录、血液领取至输注时间、双方核对、输血血袋统一回收处理9个项目，计算缺陷率和合格率。针对存在的问题护理部、输血科进行持续改进。2012年为改进前，2013年为改进中，2014年为改进后。结果：2012年75份输血病历存在护理质量缺陷，占31.25%，2013年41份输血病历护理质量缺陷率为17.08%，2014年2份输血病历护理质量缺陷率为0.83%。结论：定期开展输血护理质量检查并进行持续改进，能有效提高输血护理质量，确保输血安全。　　【关键词】 临床输血； 护理质量； 持续改进　　中图分类号 R473.5 文献标识码 B 文章编号 1674-6805（2016）27-0051-02　　Transfusion Nursing Standardized Management and Continuous Improvement of Clinical Blood/WANG Ning，GAO Jing.//Chinese and Foreign Medical Research，2016，14（27）：51-52　　【Abstract】 Objective：To analyze problems of blood transfusion nursing quality management in medical records，by strengthening the nursing quality management to promote the blood transfusion nursing standardization，improve clinical transfusion safety，avoid medical disputes.Method：Blood transfusion archive records were randomly sampled from 2012 to 2014，20 samples a month， a total of 720 samples.The nine projects of blood transfusion application，blood transfusion apps specimen，blood transfusion（matching） orders report，blood transfusion orders，blood transfusion reactions，blood transfusion nursing records，obtaining and infusion on both sides to check blood，blood transfusion bags unified recycling were examined.The defect rate and qualified rate were calculated.Aimed at the problems，the nursing department，blood transfusion department conducted continuous improvement.2012 was before improving，2013 was during improving，2014 was after improving.Result：2012 of 75 blood transfusion medical records existed different defects， ratio of 31.25%， 2013， 41 blood transfusion medical record defect rate was 17.08%， 2 blood transfusion medical record defects was 0.83% in 2014. Conclusion：Regularly carrying out blood transfusion nursing quality check and management intervention， can effectively improve the quality of blood transfusion nursing， ensure transfusion
