探析多层螺旋CT动脉期对肝海绵状血管瘤诊断价值 　 作者：刘国荣，程传虎，蓝博文，黄尧生 【关键词】 肝海绵状血管瘤；断层摄影术；X线计算机 　　［摘 要］ 目的：探讨多层螺旋CT动脉期对肝海绵状血管瘤的诊断价值。方法：回顾分析45例共59个病灶，经手术病理或肝血管造影介入治疗证实的肝海绵状血管瘤，全部病例均行多层螺旋CT平扫，动、静脉双期扫描和延期扫描。结果：59个病灶平扫均表现为圆形或类圆形低密度区。病灶动脉期表现为：病灶边缘呈结节状强化32个。病灶边缘呈斑点状强化12个。病灶边缘呈小结节并半环状强化9个。病灶边缘呈小点状强化6个。静脉期全部病灶强化区扩大。延时期55个病灶填满强化，密度等于肝实质或略高于肝实质，4个病灶中央无强化。结论：多层螺旋CT动脉期对肝海绵状血管瘤有重要诊断价值。 　　［关键词］ 肝海绵状血管瘤；断层摄影术；X线计算机 　　The Diagnostic Value of Arterial Phrase in Hepatic Scanning by Multipleslices Spiral CT to Cavernous Hemangioma of Liver 　　Abstract：Objective To investigate the diagnostic value of arterial phrase in hepatic scanning by multipleslices spiral CT to cavernous hemangioma of liver.Methods 59 focuses in 45 patients were analyzed retrospectively and confirmed as cavernous hemangioma of liver by pathological examination or the interventional therapy of liver.The arterial phase,portal vein phase,hepatic plain scan and delayed scanning in hepatic scanning by multipleslices spiral CT were performed in all cases.Results There was roundshape or roundshape like low density area in plain scanning of 59 focuses.The appearance of arterial phase in focuses displayed the nodosity enhancement in 32 focuses,the mottling enhancement in the fringe of 12 focus,the little tubercle and semiring enhancement in the fringe of 9focus,the dot enhancement in the fringe of 6 focus.The enhanced areas in all focuses expanded in portalvein phase.55 focuses were fully enhanced in delayed phase.The densities of focuses were equal to,or higher than that of liver parenchyma.There were no enhancements in the center of 4 focuses.Conclusion Arterial phrase in hepatic scanning by multipleslices spiral CT is valuable for diagnosis of cavernous hemangioma of liver. 　　Key words:Cavernous hemangioma of liver；Tomography；Xray computer tomography 　　肝海绵状血管瘤是常见病，影像工作者由于对其螺旋CT动脉期的征象不了解、不熟悉，日常工作中经常不能快速诊断。笔者查阅近期多年有关文献，虽有文献报道肝海绵状血管瘤的CT诊断，但还是沿用经典的诊断标准，而用多层螺旋CT去探讨其动脉期对肝海绵状血管瘤的诊断价值报道极少。笔者通过对45例，共59个肝海绵状血管瘤螺旋CT表现的研究，着重探讨其在动脉期的诊断价值。 　　1 资料和方法 　　1.1 一般资
