中华普通外科学文献 （电子版）20 10 年 10月第 4 卷第 5 期 Chin Arch Gen Surg （Electronic Edition），October 2010 ，Vol 4 ，No.5 ·461· · 论著 · 肝移植术后糖尿病的诊治策略 安玉玲 蔡常洁 包维民 易慧敏 陆敏强 陈规划 【摘要】 目的 探讨肝移植术后糖尿病 （PTDM ）的临床特征和诊治措施。方法 回顾性分析我 院 自2003 年 10 月到 2008 年 8 月的 58 例 PTDM 患者的临床资料。70.7% （4 1/58 ）的患者予胰岛素治 疗 ， 激素组 23 例 口服激素 的 PTDM 患者中有 15 例激素减量或停用； FK506 组 16 例使用单药他克莫 司 （FK506 ）免疫抑制方案的 PTDM 患者 ， 转换为霉酚酸酯 （MMF ）或西罗莫司 （RPM ）为主的免疫抑制方 案， 随访 3 个月， 观察改变免疫抑制方案对 PTDM 的影响。 结果 89.7% （52/58 ） 的 PTDM 患者临床症状 不典型， 激素组有 5 例患者空腹血糖正常， 出现糖尿病逆转； 11 例患者胰 岛素用量显著降低或停用， 无 一例发生急性排斥反应。FK506 组有 2 例患者糖尿病逆转， 10 例患者胰岛素用量显著降低或停用 ， 1 例 患者发生急性排斥反应， 在 FK506 加量后排斥反应得以控制 。结论 肝移植术后 PTDM 的临床症状不 典型， 胰 岛素治疗占主体。减低激素和 FK506 用量 ， 转换 MMF 或 RPM 为主的免疫抑制方案， 可能是防 治 PTDM 的有效手段。 【关键词】 肝移植； 糖尿病； 免疫抑制剂 ； 治疗 Management of diabetes mellitus following orthotopic liver transplantation A N Yu-ling, CA I Chang- j ie, BA O We i-min, YI H ui-min, L u Min -q iang, CHEN Gui -hua.
