精品论文 参考文献 左卡尼汀联合高钙透析液对老年血液透析患者心血管稳定性的作用 北京老年医院 北京市 100095 摘要：目的 研究左卡尼汀注射液联合高钙透析液对老年血液透析患者心血管稳定性的作用。方法 将80例老年血液透析患者随机分为对照组和治疗组，每组40例。两组患者均给予透析治疗，对照组患者采用1.25%钙浓度透析液，治疗组患者采用1.75%钙浓度透析液。治疗组患者每次透析后加用左卡尼汀1.0g静脉注射。比较两组患者3月治疗期间心血管并发症的发生率以及治疗6月后甲状旁腺、血浆肾素、血管紧张素Ⅱ的浓度。结果治疗3月期间治疗组患者心血管并发症较对照组明显减少（Plt;0.05）；对照组患者治疗后各激素较治疗前略有下降，但差异无统计学意义（P gt;0.05）；治疗组治疗6月后各激素较治疗前明显下降，差异有统计学意义（P lt;0.05），并显著低于对照组（P lt;0.05）。结论 左卡尼汀联合高钙透析液可增加老年血液透析患者心血管稳定性，有效清除甲状旁腺素、肾素及血管紧张素Ⅱ。 关键词：左卡尼汀；高钙透析液；血液透析；心血管并发症 [Abstract] objective To investigate the effects of L-carnitine and high calcium concentration in dialysate in cardiovascular stability in elderly patients with haemodialysis. Methods Eighty elderly patients with haemodialysis were randomly divided into two groups，40 in each group. Both of groups were given to haemodialysis treatment，besides 1.25% calcium concentration in dialysate in control group and 1.75% calcium concentration in dialysate in treatment group. L-carnitine was infused to patients in treatment group at the end of dialysis treatment for 12 weeks. The incidence of cardiovascular complications in two groups during dialysis，and parathyroid hormone，plasma rennin，angiotensin Ⅱ changes after 6 months of treatment were compared. Results Cardiovascular complications rate of patients in control group was significantly reduced compared with that in reatment group during dialysis（Plt;0.05）. There were no differences of parathyroid hormone，plasma rennin，angiotensin Ⅱ before and 6 months after treatment in control group（P gt;0.05）. Moreover，these hormones were found decreased in reatment group compared with these in control group and before treatment. Conclusion Elderly patients of haemodialysis with L-carnitine and high calcium concentration in dialysate have better cardiovascular stability and more effective removal of parathyroid hormone，plasma rennin and angiotensin Ⅱ.
