不同时辰中药保留灌肠对慢性肾功能不全疗效影响的护理研究摘要目的探讨不同时辰中药保留灌肠对药物保留时间及灌肠疗效的影响。方法将60例慢性肾功能不全患者按入院先后顺序随机分为观察组(30例)和对照组(30例)。观察组在辰时(7 ：00～9 ： 00)进行灌肠，对照组在常规时问戌时(19 ： 00～21：00)进行灌肠。比较两组每天灌肠前排便率、灌肠后保留时间以及灌肠后的疗效。观察周期为15 d(一个疗程)。结果灌肠前平均排便率观察组为83．6％，对照组灌肠前平均排便率为18．3％，差异有显著意义(P0．01)；平均保留时间观察组为(828．22士341．87)rain，对照组为(558．23士164．65)rain，差异有显著意义(P=0．001)；观察组总有效率为92．3％，对照组总有效率为78．6％，差异有显著意义(P一0．009)。结论不同时辰中药保留灌肠可以增加药物在肠道内的保留时间并可提高中药保留灌肠的疗效。关键词慢性肾功能不全时辰灌肠保留时间护理Abstract 0bjective To study the impact of retention enema by Chinese traditional Medicine at different hour．Method 60 patients with pelvic inflammatory disease were randomly divided into observation group(30 cases)andcontrol group(30 eases)．The observation group practice enema at 7}00—9：00 comparing with the control group atroutine hour(19 1 00—2l l 00)．The percentage of bowel movement before enema，the retention time and the curativeeffects for 1 5 days(one period)was compared between the tWO groups．Result The average percentage of bowelmovement in observation group(83．6％)was much higher than that of in control group(18．3％)(PO．01)．Theaverage retention time was(828．22±341．87)rain in observation group compared with(558．23±164，65)rain incontrol group，there was significant difference between the tWO groups(P=0．001)．The gross percentage of validityin observation group(92．3％)was higher than that of in control group(78．6％)(P=0．009)．Based on theSpearman and Kendall correlation coefficients analysis between the efficacy and retention time，There was significantcorrelation between effects and retention time(PO．01)．Conclusion Practice retention enema at different time canincrease the retention time and curative effects of retention enema by Chinese traditional Medicine．Key words Pelvic Inflammatory disease Hour Enema Retention time Nursing慢性肾功能衰竭（CRF）是各种肾脏疾病终末期的共同表现，是由于肾单位的严重破坏造成机体排泄代谢废物功能减退，水、电解质、酸碱平衡紊乱，以及某些内分泌功能异常的临床综合征[1]。属于祖国医学关格癃闭水肿虚劳等范畴。中药保留灌肠疗法是非透析的传统治疗方法之一，能阻抑或减少毒素的产生，加速体内有毒物质的排泄，对解决慢性肾功能不全患者的早期治疗，延缓病程的发展，延长慢性肾功能不全者的寿命具有重要的意义，是常用中医护理操作之一。研究表明。中药
