精品论文 参考文献 探讨慢支肺气肿合并真菌感染的临床效果观察 尚志市河东乡卫生院 150600 【摘 要】目的：研究分析慢支肺气肿合并真菌感染的临床疗效观察。方法：此次研究的对象是选择医院2013年10月至2014年10月收治的慢支肺气肿合并真菌感染患者67例，将其临床资料进行回顾性分析，并给予患者药物治疗，并取患者痰液进行真菌培养，观察临床治疗效果以及真菌培养结果。结果：经过治疗之后63例患者好转，4例患者出现轻度呼吸衰竭，6例患者发生并发症。真菌培养结果：白色念珠菌52例、热带念珠菌21例、光滑念珠菌19例、曲霉菌属14例、毛霉菌属7例、克柔念珠菌5例，两种以上念珠菌混合感染47例。结论：临床治疗慢支肺气肿合并真菌感染患者时，应及时进行痰液真菌检查，尽早给予患者对应治疗，提升治疗效果。 　　【关键词】慢支肺气肿；真菌感染 　[Abstract] Objective：To observe the clinical curative effect of chronic bronchitis and emphysema complicated with fungal infection. Methods：the object of the study is choice of hospital from October 2013 to 2014，the October of chronic bronchitis and emphysema complicated with fungal infection in patients with 67 cases，their clinical data were retrospective analysis，and patients were given drug treatment，and patients with sputum fungal culture，clinical observation treatment curative effect and fungal culture results. Results：after treatment，63 patients improved，4 cases of patients with mild respiratory failure，6 cases of complications. Fungus culture results：52 cases of Candida albicans，21 cases of Candida albicans，19 cases of Candida albicans，14 cases of Aspergillus，7 cases of Candida，5 cases of Candida，two cases of mixed infection of 47 cases. Conclusion：the clinical treatment of chronic bronchitis and emphysema complicated with fungal infection patients should be timely sputum fungal examination and receive with the corresponding treatment as soon as possible and improve the therapeutic effect. [keyword] chronic bronchitis；fungal infection 随着医疗水平的提升，临床治疗中越来越多地应用激素类、抗菌药物、免疫抑制剂等，导致真菌感染发病率提高。慢支肺气肿为慢性肺部疾病，患者住院期间比较容易发生真菌感染，增加治疗难度，影响患者预后，本研究以67例慢支肺气肿合并真菌感染患者为研究对象，给予对应治疗，探讨临床治疗体会，现报告如下。 　　1 资料与方法 　　1.1 一般资料 　　选择医院2013年10月至2014年10月收治的慢支肺气肿合并真菌感染患者67例，男34例，女33例；年龄25～82岁，平均（51.3plusmn;2.4）岁。纳入标准：①所有患者符合慢支肺气肿及真菌感染诊断标准；②经X线片检查及痰菌检查确诊；③3个月内患者未服用广谱或联合抗菌药物；④排除心、肝、肾功能不全，药物过敏患者。 　　1.2 方法 　　治疗方法：积极治疗患者慢支肺气肿，给予患者沙丁胺醇气雾剂联合异丙托溴铵气雾剂联合治疗，每次1～2喷，每天3～4次；给予患者真菌感染治疗，静脉滴注氟康唑注射液200mg，首次加倍，每天1次，持续8～10d，随后给予患者氟康唑片100mg，口服，
