精品论文 参考文献 早孕期剖宫产瘢痕妊娠的临床及病理特点分析 袁世强 袁世强１　辛海２ 通讯作者１．无锡妇幼保健院　江苏　无锡　２１４０００;２．无锡妇幼保健院　江苏　无锡　２１４０００ 　　【摘要】　目的　针对早孕期剖宫产瘢痕妊娠的临床及病理特点进行研究探讨.方法　选自２０１３年１月~２０１５年１月这一期间,７８例在本院进行分娩的剖宫产瘢痕妊娠患者作为本次的研究对象,将患者留档的活检样本进行病理切片研究,对患者的临床和病理学特点进行了解.结果　经过本次研究结果显示,患者的平均年龄为(３３．３９plusmn;５．２９)岁,患者的平均停经时间为(５２．０２plusmn;９．６０)d,阴道出血是主要的临床首发病症,外生型患者的妊娠频率、手术时间、出血量、HCG含量、身体恢复时间等指标均高于内生型患者,但是停经时间短于内生型患者,两组对比差异具有统???学意义(P＜０．０５),但是内生型患者和外生型患者的年龄以及HCG恢复正常时间对比差异无统计学意义(P＞０．０５).结论　从临床和病理学特点出发,内生型患者和外生型患者的部分数据具有明显不同,造成瘢痕妊娠的主要原因与瘢痕所处肌层具有缝隙有着紧密相关的联系.利用超声影像学检查可以准确判断患者的病情,为治疗提供准确的数据. 【关键词】　早孕期;剖宫产瘢痕妊娠;病理学;临床特点;分析【Abstract】　Objective:Toinvestigatetheclinicalandpathologicalcharacteristicsofcesareanscarpregnancyinearlypregnancy．Methods:７８casesofcesareanscarpregnancyinourhospitalduringtheperiodfromJanuary２０１３toJanuaryinwereselectedastheresearchobjects．Thebiopsysampleswerecollectedfromthepatientstounderstandtheclinicalandpathologicalcharacteristicsofthepatients．Results:afterthestudyresultsshowthat,patientswithanaverageageof(３３．３９＋５．２９)years,mediantimetomenopausefor(９．６０５２．０２＋ D),vaginalbleedingisthemaininitialclinicaldisease,patientswithexophyticpregnancyrate,operationtime,bleedingindex,levelsofhCG,bodyrecoverytimewerehigherthantypepatients,butmenopausetimeisshortintypepatients,differencesbetweenthetwogroupshasstatistiＧcalsignificance(P＜０．０５),butthestudentstypepatientsandexogenouspatientsageandhCGrestorednormaltimedifferencethereisnostatisticalsignificance(P＞０．０５)．Conclusion:accordingtotheclinicalandpathologicalcharacteristics,theinternalandexternaltypesofpatientswiththedatahassignificantlydifferent,resultinginthemaincauseofscarpregnancyandthescariscloselyrelatedtothemusclelayerhasaclosecorrelation．Ultrasoundimagingcanaccuratelyjudgethepatient＇scondiＧtionandprovideaccuratedatafortreatment．【Keywords】　Earlypregnancy;cesareanscarpregnancy;Pathology;clinicalcharacteristics;analysis 【中图分类号】R４７８ 【文献标识码】A 　　【文章编号】１００８－６３１５(２０１５)１２－０１２９－０１　　 　　在产科的医学领域中,剖宫产妊娠是即为罕见的剖宫产远期并发症[１],该病属于异位妊娠的一种,是指受精卵、孕囊或者胚胎着床与患者剖宫产手术切口的瘢痕上,该并发症可到能
