精品论文 参考文献 植入式静脉输液港在乳腺癌化疗患者中的应用评价 刘媛 归奕飞 廖文勇 黄燕萍 刘凌丽 韦佩沂 王楠楠 　　【摘 要】 目的 评价植入式静脉输液港在乳腺癌化疗患者中的应用价值。方法 回顾性分析植入式静脉输液港在柳州市工人医院普外四病区自2012年5月至2015年7月收住的356名乳腺癌化疗患者中的应用，以常规中心静脉置管（80例）为对照，chi;2检验或Fisher精确检验分析比较两组患者留置导管期间生活质量评分优良率及并发症发生率有无统计学差异。结果 与常规中心静脉置管相比，接受植入式静脉输液港植入的患者生活质量更好（chi;2=6.25，P=0.01），非计划拔管、血栓形成、穿刺处感染、导管相关血液感染、导管断裂/移位、导管功能障碍的发生率均较低且差异具有统计学意义（P＜0.05）气胸发生率则无统计学差别。结论 植入式静脉输液港术后患者生活质量高，并发症少，优于传统中心静脉置管，是乳腺癌化疗患者安全、方便、舒适的输液途径。 　　【关键词】 植入式静脉输液港； 乳腺癌； 化疗 　　 　　Application evaluation of implantable venous-access port in chemotherapy patients with breast cancer Liu Yuan，Gui Yi-fei，Liao Wen-yong，Huang Yan-ping，Liu Ling-li，Wei Pei-yi，Wang Nan-nan. Department of Mammory Surgery, Liuzhou Workerrsquo;s Hospital of Guangxi Province, Liuzhou 545005, China 　　 　　【Abstract】 Objective To evaluate implantable venous-access port (IVAP) in breast cancer patients with intravenous chemotherapy. Methods Totally 436 breast cancer patients who underwent chemotherapy in our department from May 2012 to July 2015 were divided into CVC （Central venous catherization） group (n =80) and IVAP group (n =356) according to the will and economic condition of the patients . Complication rate and quality of life during intravenous chemotherapy were compared. All data were analyzed using Chi-square test or Fisherrsquo;s exact test. Results　 Compared with IVAP group, the quality of life of patients in CVC group declined significantly（chi;2=6.25，P=0.01）. Incidence rates of unplanned tube drawing ，thrombogenesis ，infection at the puncture location，catheter-related bloodstream infections，catheter malfunction，catheter fracture/shift in IVAP group were all lower than those in CVC group（p＜0.05） ，while the incidence rates of pneumothorax in these two groups did not reach statistical significant difference. Conclusion　IVAP brings fewer complications and less impact on life quality of patients, which may be the better choice for intravenous chemotherapy in breast cancer patients . 　　【Key words】
