以下备注为填写申请书(A.M.FISS)提供指引。 The following notes give guidance on how to complete the application form (A.M. FISS) (a) 申请人只能就此次项目填写一份申请书。 Applicant should complete one application form for this program. (b) 申请人须填妥申请书各项，并提供正确数据。如空位不敷填写，申请人应另页详列有关资料，随申请书附上。 Please ensure that all parts in the form are completed and the information is accurate. If there is insufficient space, please give details on a separate sheet to be attached to the application form. (c ) 申请人必须填报申请书内要求提供的所有个人资料(注明是可选择是否填报的数据则属例外)，否则申请书将不获受理。 Your provision of all the personal data requested in this form is obligatory(except those items clearly marked as optional). You should particularly note that program may require you to provide specific details to support your application for individual vacancies. Your application will not be considered if you fail to provide all information as requested or it is not clearly. (d) 申请人在本申请书内所提供的个人资料，将用于项目以及其他与有关的事宜上。如有需要，有关资料可能会送交获授权处理有关数据的部门及其他组织或机构，用以进行与项目有关的事宜。在一般情况下，未获取录申请人的数据将于其落选日期后12个月全部销毁。 The personal data provided in this form will be used for the program and other program-related purposes. It may be provided to related departments and other organizations or agencies authorized to process the information for necessary purposes.
