综合管理对慢性心力衰竭患者心功能及生活质量影响 　　摘 要 目的：了解社区综合管理对慢性心力衰竭患者心功能及生活质量的改善作用。方法：采用抽签法选择慢性心力衰竭患者100例，随机分为2组各50例，综合干预组除予以病因及抗心力衰竭治疗外，增加健康教育、行为、运动、营养、心理、增强免疫力、指导用药等7项干预措施；对照组只给予病因及常规抗心力衰竭治疗。每月进行心功能、生活质量、服药依从性、再入院率评估，随访观察6个月，比较干预前后心功能及生活质量的变化。结果：干预后干预组患者的心功能、生活质量、服药的依从性明显提高，再入院率、死亡率显著降低，与对照组比较差异有统计学意义(P＜0.05)。结论：社区综合管理是治疗和预防慢性心力衰竭的有效方法，能有效改善心功能，提高生活质量，值得在社区应用推广。 　　关键词 慢性心力衰竭 心功能 社区综合管理 生活质量 再入院率 　　中图分类号：R541.6+1 文献标识码：A 文章编号：1006-1533（2012）06-0043-03 　　Effect of the comprehensive management on the cardiac function and 　　life quality of the patients with chronic heart failure 　　TANG Wei, LIU Qin, CHEN Li-ping, ZHOU Mei-feng, JIN Xue-juan, ZHOU Jing-min, ZHOU Jun 　　(Jiading Community Health Service Center, Shanghai, 201800) 　　ABSTRACT Objective：To learn the role of the community comprehensive management so as to improve the cardiac function and life quality of the patients with chronic heart failure. Method：100 cases with chronic heart failure were randomly divided into 2 groups of 50 patients each. The comprehensive intervention group, in addition to etiologic and CHF treatment, added health education, behavior, exercise, nutrition, psychology, increasing immunity, instructing medication interventions; the control group was only given the etiology and conventional heart failure therapy. Monthly the cardiac function, life quality, compliance, rehospitalization rate evaluation were followed up for 6 months and the results were compared and observed before and after comprehensive intervention. Results： The results of the intervention group had remarkable improvement in cardiac function, life quality, compliance and obviously reduced re-hospitalization rate and mortality rate. Compared with the control group it had statistic significance ( P＜0.05 ).
