中医药治疗老年脑梗死患者抑郁症状与血液流变学改变关系 　　 摘 要：目的：观察中医药治疗老年脑梗死患者抑郁症状与血液流变学改变的关系。方法：收集42例老年脑梗伴抑郁症状患者分别于中医药治疗前和3周后进行血液流变学检测和比较，同时评定汉密尔顿抑郁量表（HAMD），并与40例老年脑梗不伴抑郁症状患者及40例健康老人进行对照。结果：①入组时老年脑梗死患者的抑郁症状（HAMD量表分）全血黏度（120s-1、70 s-1和30 s-1）、与全血还原黏度（高切、中切和低切）、血浆黏度、红细胞聚集指数、红细胞压积等指标都呈显著正相关（P＜0.05~0.01）;②伴抑郁组42例（HAMD总分＞17分）与40例不伴发者比较，伴发组的全血黏度（120s-1、70 s-1和50 s-1）、全血还原黏度（高切、中切和低切）、血浆黏度、红细胞压积等指标都显著高于对照（非伴发组和健康组，P＜0.01），通过中医药治疗后这种差异减轻甚至消失（P＜0.05~0.01）；③伴抑郁组全血黏度（120 s-1、70s-1和30s-1）、与全血还原黏度（高切、中切和低切）、红细胞聚集指数、红细胞压积等指标经中医药治疗3周的变化值与抑郁量表的减分值呈显著正相关（P＜0.05~0.01）。结论：老年脑梗死患者抑郁症状与血液流变学改变的关系密切，伴抑郁症状的老年脑梗患者的高黏滞血症更突出，其高黏滞的程度与抑郁症状平行，通过动态观察血液流变学改善情况与疾病好转相关程度，为中医药治疗老年脑梗死患者抑郁症状的疗效提供了现代科学依据。 　　 关键词：脑梗死；血液流变学；抑郁；高黏滞血症 　　中图分类号：R743.33 文献标识码：B 文章编号：1673-7717(2012)01-0032-03 　　TCM Therapy For Elderly Patients With Cerebral Infarction Depressive Symptoms And The Relationship Between The Change Of Bloodrheology 　　JIA Chun?xia1, ZHANG Yu?yan2，WANG Zheng?liang1, SHEN Chun?yan1 　　(1.Zhejiang Care Hospital, Hangzhou 310005,Zhejiang,China; 　　2.Zhejiang Chinese Medicine University, Hangzhou 310053,Zhejiang,China) 　　Abstract:Objective: To observe the TCM therapy in elderly patients with cerebral infarction depressive symptoms and the relationship between the change of blood rheology.
