吡拉西坦对高血压脑出血术后患者的临床疗效 王俊 王超 杨承勇 张勇 王曲 马俊 （贵州省人民医院神经外科，贵州 贵阳 550002） 【摘要】　目的 探讨吡拉西坦对高血压脑出血术后高颅压及神经功能恢复的治疗效果。方法 将146例高血压脑出血术后患者随机分为吡拉西坦治疗组（治疗组）和甘露醇治疗组（对照组），观察治疗早期患者的意识恢复情况、颅内压变化、脑水肿程度；应用巴氏指数（Barthel index, BI）和简明精神状态检查量表（mini mental state examination, MMSE），评价两组患者远期的日常生活活动能力和认知功能。结果 降低颅内压的效果组间差异无统计学意义（p＞0.05）；治疗组1周内的意识清醒率、脑水肿的程度、3个月后BI及MMSE均显著优于对照组（p＜0.05）。结论 吡拉西坦具有与甘露醇相似的降低颅内压的作用，并能促进高血压脑出血患者神经功能的恢复，改善认知功能障碍，提高患者的生存质量。 【关键词】 吡拉西坦；高血压脑出血；颅内压；疗效 【中图分类号】R 743．34 【文献标识码】A The clinic efficiency of Piracetam on the postoperative patients with hypertensive intracerebral haemorrhage Wuang Jun WANG Cao YANG Chengyong ZHANG Yong WANG Qu MA Jun Department of neurosurgery, GuiZhou province people’s hospital, Guiyang 550002, China 【abstract】 objective To investigate the therapeutic efficiency of Piracetam on postoperational high intracranial pressure and neurofuctional rehabilitation for the patients wtih hypertensive cerebral hemorrhage.Methods 146 postoperational cases of hypertensive cerebral hemorrhage were randomly divided into Piracetam therapeutic group (therapeutic group) and mannitol therapeutic group (control group) . the state of consciousness rehabilitation, the variation of intracranial pressure , the degree of cerebral edema were observed in early therapeutic stage . the scores of Barthel index ( BI） and mini mental state examination (MMSE）were used to assesse the activities of daily life and the cognitive ability .Results No statistical significant difference was found in two group in reduction of intracranial pressure（p＞0.05） . the rate of clear consciousness and the condition of cerebral edema in a week , scores of BI andMMSE after three months（p＜0.05）in the therapeutic group was significantly better than the control group.Conclusion Piracetam had same effect with mannitol on reduction of intracranial pressure ,the Piracetam aid in the rehabilitation of neurofunction for postoperational patients with hypertensive cerebral hemorrha
