DSM-IV-TRDiagnostic and Statistical Manual of Mental Disorders(4th ed., text revision): A brief overview The Manual as a Resource It is used across several disciplines – clinicians, researchers, psychiatrists, psychologists, social workers, and counselors, just to name a few. It is used across different settings – hospitals, private practice, inpatient, outpatient, etc. How does it impact school counselors? – School counselors do not diagnose the student son their caseload; however, they often work with students that are being treated for a diagnosis. It is important for school counselors to be aware of and familiar with the terminology in the DSM-IV TR. Multiaxial System The DSM-IV uses a multiaxial classification system that includes 5 axes: Axis I Clinical Disorder Axis II Personality Disorders Mental Retardation Axis II General Medical Conditions Axis IV Psychosocial and Environmental Problems Axis V Global Assessment Functioning Why? This multiaxial system allows for a comprehensive and systematic evaluation of an individual. Axis I: Clinical Disorders Axis I is for reporting all clinical disorders except for personality disorders and mental retardation. It is possible that there is no Axis I disorder or that it is Deferred until further information can be gathered. Also, it is possible for more than one clinical disorder to be listed; however, the principal diagnosis should be listed 1st. Axis I: Clinical Disorders A few examples: Schizophrenia Major Depressive Disorder Bipolar I Disorder Anxiety Disorder NOS Adjustment Disorder Obsessive Compulsive Disorder Axis II: Personality Disorders/ Mental Retardation Axis II is for reporting Personality Disorders and Mental Retardation. It is possible for there to be no Axis II disorder, to Defer, and it is possible for there to be more than one. Some examples of personality disorder
