双胎妊娠剖宫产术中预防性行改良B―lynch缝合术对产后出血及产妇预后影响 　　[摘要] 目的 研究预防性改良B-lynch缝合术对双胎妊娠剖宫产产妇产后出血及预后的影响。 方法 选取2014年1月～2015年6月在徐州医科大学附属徐州市妇幼保健院因双胎妊娠行剖宫产的产妇109例，随机分为两组，其中观察组50例，术中采用B-lynch缝合术，对照组59例，采用常规方法处理，观察两组中术中出血量，术后24h出血量，手术时间，缩宫剂使用量，产褥病发生率及恶露持续时间。 结果 观察组、对照组产妇术中的出血量、术后24h内出血量及手术时间比较，差异有统计学意义（P0.05）。 结论 双胎妊娠剖宫产术中预防性行改良B-lynch缝合术，对预防和治疗产后出血效果显著，缩短了手术时间，减少了宫缩剂的用量，不增加手术并发症的发生，可作为双胎妊娠剖宫产术中常规术式进行。 　　[关键词] 双胎妊娠；剖宫产术；改良B-lynch缝合术；产后出血 　　[中图分类号] R719.8 [文献标识码] B 文章编号] 2095-0616（2015）24-85-04 　　[Abstract] Objective To research the effect of preventive improved B-lynch suture technique on postpartum hemorrhage ang prognosis in patients with pregnancy cesarean section. Methods From January 2014 to June 2015 in the Maternal and Child care service center of Xuzhou City， 109 cases with the twin pregnancy cesarean section maternal were selected as the research object， and randomly divided into two groups. There were 50 cases in the observation group， received B-lynch suture technique group； 59 cases in the control group， dealed with the conventional methods. The blood volume during the surgery and 24 hours after surgery， duration of operation， the requirement of oxytocin and Romethamine injection， the incidence of puerperal disease， the duration of lochia were observed. Results Compare the bleeding volume in operation， the amount of bleeding within 24 hours after surgery and the operation time between the treatment group with the control group， the difference was statistically significant （P0.05）. Conclusion Improved B-lynch suture in cesarean section of twin pregnancy， can significantly prevent and treat postpartum hemorrhage， shorten the operation time， reduce the amount of uterotonics， and it does not increase the incidence of surgical complications.
