异常增量用药联合干预机制在我院抗菌药物合理使用中作用 　　[摘要] 目的 探讨异常增量用药的联合干预机制在我院抗菌药物合理使用中的作用。 方法 设置异常增量预警标准，选取2013年1～12月的相关数据作为干预前对照，选取2014年1～12月的相关数据作为干预后结果。对2014年采用异常增量监测和专项处方点评的数据方法进行跟踪分析。药剂科对不合理处方的原因汇总分析，提出合理用药建议。医务科对不合理用药行为采取通报和考核等行政干预手段。 结果 与2013年比较，2014年的联合干预数据结果显示，本院的抗菌药物临床使用各项指标日趋合理，其中门、急诊抗菌药物使用率和围手术期Ⅰ类切口预防使用抗菌药物使用率环比下降3.3%、16.8%、54.9%，均达到了原卫生部的相关指标要求。塞克硝唑片、奥硝唑胶囊及注射用头孢噻肟的用量排名和金额占比均有所下降。 结论 异常增量用药的联合干预机制可作为促进临床合理用药的有效手段，其对不合理用药行为具有明显的遏制作用。 　　[关键词] 异常增量；联合干预；抗菌药物；合理使用 　　[中图分类号] R978.1 [文献标识码] A [文章编号] 1674-4721（2016）03（c）-0087-05 　　[Abstract] Objective To explore the role of the combined intervention mechanism of abnormal incremental dosage in the rational use of antibiotics in our hospital. Methods The abnormal incremental dosage warning criteria was set firstly，then the relative data from January to December in 2013 were selected as the control group before intervention，the relevant data from January to December in 2014 were selected as the result after intervention.The data and methods of using monitoring of the abnormal incremental dosage and special review in 2014 were tracked and analyzed，the causes of irrational prescription were summed and analyzed by department of pharmacy，and suggestions for the unreasonable prescription were proposed.Administrative intervention measures such as notification and examination were taken for the irrational drug use behavior by medical services section. Results Compared with 2013，the combined intervention data results showed that the index of clinical application of antibiotics in our hospital was more reasonable in 2014，among them，the use rate of antibiotics in the outpatient and emergency department，the use rate of antibiotics of preventive in the Ⅰ cut perioperative class decreased by 3.3%，16.8%，54.9%，which had reached the relevant requirements of the former ministry of health.The dosage ranking and the proportion of expenses of secnidazole tablets，ornidazole capsules and injection of cefotaxime had declined.
