心理干预在分娩镇痛中应用 　　【摘要】 目的：探讨心理干预在分娩镇痛中的应用。方法：将笔者所在医院计划无痛分娩400例初产妇随机分为干预组和对照组。对照组进行常规干预，干预组在此基础上进行心理干预，比较两者产妇镇痛效果、镇痛满意度、分娩镇痛时间、分娩方式、胎儿宫内窘迫率、新生儿Apgar评分、新生儿窒息率。结果：干预组产妇镇痛效果和镇痛满意度优于对照组，干预组的剖宫产率低于对照组，分娩镇痛时间少于对照组，差异均有统计学意义（P0.05）。结论：心理干预应用于分娩镇痛能降低剖宫产率、明显提高产妇的舒适度和满意度、促进医患关系，值得在分娩镇痛过程中广泛开展。 　　【关键词】 心理干预； 分娩镇痛； 胎儿宫内窘迫 　　中图分类号 R473.71 文献标识码 B 文章编号 1674-6805（2016）5-0024-03 　　【Abstract】 Objective：To study the effect of psychological intervention on labor analgesia.Method：400 unipara who decide to receive labor analgesia were randomly divided into the experiment group and the control group.The control group was given the convention intervention，the intensive psychological intervention was given to the experiment group on the basis of the control group.The analgesic effect，the satisfaction of analgesic，the time of labor analgesia the delivery，and the occurrence rate of fetal distress，Apgar scores and the occurrence rate of neonatal asphyxia were observed.Result：The analgesic effect and satisfaction of analgesic in the experiment group were better than those in the control group.The cesarean section rate in the experiment group was lower than that in the control group.Meanwhile，the time of labor analgesia in the experiment group was shorter than that in the control group（P0.05）.Conclusion：Psychological intervention used in labor analgesia is able to reduce cesarean section rate，enhance comfort level and satisfaction of maternity and promote the doctor-patient relationshPsychological intervention is worthy of using in labor analgesia process extensively. 　　【Key words】 Psychological intervention； Labor analgesia； Fetal distress 　　First-author’s address：Maternal and Child Health Care Hospital of Changsha，Changsha 410007，China 　　doi：10.14033/j.cnki.cfmr.2016.5.012 　　随着产妇及家属对分娩舒适度要求越来越高和社会意识的转变，减轻产妇分娩痛苦已经由减轻个体疼痛上升到尊重人权和生命、彰显社会文明的高度。硬膜外镇痛通过抑制中枢神经系统兴奋性递质释放和神经冲动的传导，改变离子通道的通透性而产生镇痛作用。因其实施方便、效果确切、不良反应少、可控性强已成为临床上应用最多的分娩镇痛方式[1]。为评估心理干预在分娩镇痛中的作用，更好发挥硬膜外镇痛的作用，笔者所在科对接受分娩镇痛的400名初产妇进行了心理干预，
