纤维支气管镜在感染性肺不张患者中应用 　　【摘要】 目的 探讨感染性肺不张患者中纤维支气管镜（纤支镜）的临床应用价值。方法 82例实施机械通气的感染性肺不张患者行纤支镜吸痰和肺泡灌洗治疗2～6次， 复查胸部X线了解肺不张演变情况， 分别记录治疗前、每次纤支镜治疗后8 h的血气分析、临床症状缓解情况。结果 59例临床症状及血气分析指标治疗后明显改善， 成功撤离呼吸机；13例实验结束时未能撤离呼吸机， 但临床症状和血气指标改善；10例无效。治疗后患者氧分压（PO2）、二氧化碳分压（PCO2）及动脉血氧饱和度（SaO2）均较治疗前明显改善， 差异有统计学意义（P0.05）。结论 纤支镜在感染性肺不张患者中的应用可发挥重要作用， 值得临床应用。 　　【关键词】 纤维支气管镜；机械通气；肺不张 　　【Abstract】 Objective To investigate clinical application value of fiber bronchoscope in infectious pulmonary atelectasis patients. Methods A total of 82 infectious pulmonary atelectasis patients， who received mechanical ventilation， took 2～6 times of fiber bronchoscope for sputum suction and alveolar wash. Chest X-ray in reexamination was taken to show changes of pulmonary atelectasis. Records were made on blood gas analysis and relieved clinical symptoms before treatment and in 8 h after each fiber bronchoscope treatment. Results There were 59 cases with remarkable improvements in clinical symptoms and blood gas analysis index after treatment， and they received successful withdrawal of breathing machine. Another 13 cases received no withdrawal， while their clinical symptoms and blood gas index were improved. There were the other 10 cases without any effectiveness. Comparing with those before treatment， all patients had obviously improved partial pressure of oxygen （PO2）， partial pressure of carbon dioxide （PCO2）， and arterial oxygen saturation （SaO2） after treatment. Their difference had statistical significance （P0.05）. Conclusion Application of fiber bronchoscope shows important effect in infectious pulmonary atelectasis patients， and it is worthy of clinical application. 　　【Key words】 Fiber bronchoscope； Mechanical ventilation； Pulmonary atelectasis 　　纤支镜自1967年正式应用于临床以来， 其在临床上的应用越来越广泛， 其适应证亦为之扩展。纤支镜在危重患者呼吸道急症中的应用， 多为挽救生命的关键性措施， 值得临床重视[1]。为探讨纤支镜在感染性肺不张患者中的应用价值， 本临床研究分析了本院2010～2012年入住ICU的82例实施机械通气的感染性肺不张患者， 现报告如下。 　　1 资料与方法 　　1. 1 一般资料 选取梅州市人民医院2010年1月～2012年12月重症医学科收治经X光或者胸部CT明确诊断为感染性肺不张患者82例， 其中男48例， 女34例， 平均年龄58
