腰―硬联合麻醉和全身麻醉在老年患者股骨颈骨折手术中麻醉效果分析 　　[摘要] 目的 探讨腰-硬联合麻醉和全身麻醉在老年患者股骨颈骨折手术中的麻醉效果。方法 整群选取该院于2015年9月―2016年4月收治的96例老年股骨颈骨折患者，按照手术的顺序随机分为实验组和对照组，每组48例患者。其中，对照组患者通过全身麻醉方式进行麻醉，实验组患者通过腰-硬联合麻醉方式进行麻醉，对比两组患者的镇痛效果以及不良反应情况。结果 实验组患者镇痛的有效率明显高于对照组，组间差异有统计学意义（P=0.037，χ2=4.360）。实验组患者不良反应率明显低于对照组，组间差异有统计学意义（P=0.028，χ2=4.80）。结论 在老年患者股骨颈骨折手术中，腰-硬联合麻醉以及全身麻醉的麻醉效果都比较理想，但是腰-硬联合麻醉的镇痛效果更明显，且不良反应发生的情况也比较少，所以，值得在临床上推广腰-硬联合麻醉。 　　[关键词] 腰-硬联合麻醉；全身麻醉；老年患者；股骨颈骨折手术；麻醉效果 　　[中图分类号] R614 [文献标识码] A [文章编号] 1674-0742（2016）11（c）-0090-03 　　[Abstract] Objective Explore the waist hard joint anesthesia and general anesthesia in elderly patients with femoral neck fracture surgery anesthesia effect. Methods Group select our hospital in September 2015 to April 2016 treated 96 cases of elderly patients with femoral neck fracture， according to the order of operation were randomly divided into experimental group and control group， each group of 48 patients. Among them， the control group patients by general anesthesia for anesthesia， the experimental group patients through the waist - hard anesthesia combined anesthesia， compared the analgesic effect and adverse reaction of two groups of patients. Results Experimental group patients and efficient of analgesia was obviously higher than that of control group， the clear difference between groups （P=0.037，χ2=4.360）. Adverse reaction rate is significantly lower than the control group， experimental group patients clear difference between groups （P=0.028，χ2=4.80）.
