超微经皮肾镜碎石术治疗肾结石术后应激肾功能变化分析 　　【摘要】 目的：分析超微?皮肾镜碎石术（UMP）治疗肾结石的术后应激、肾功能指标变化。方法：选取本院2015年4月-2016年10月100例肾结石患者为研究对象，按照随机数表法分为观察组和对照组各50例，观察组采用UMP术治疗，对照组采用经皮肾镜取石术（PCNL）进行治疗，比较两组手术时间、术中出血量、住院时间、结石清除率、术后并发症；分析白细胞介素6（IL-6）、白细胞介素10（IL-10）、白细胞（WBC）、C反应蛋白（CRP）及皮质醇（COR）、血肌酐（Cr）、血尿素氮（BUN）变化。结果：观察组与对照组比较，手术时间和住院时间短，出血量少，结石清除率高，差异均有统计学意义（P0.05）；观察组术后1、3、5 d血清IL-6、IL-10、WBC、CRP、COR均显著低于对照组相同时点的水平，比较差异均有统计学意义（P0.05）。结论：UMP治疗肾结石手术时间短，术中出血量少，结石清除率高，术后应激小，且对肾功能无明显影响，安全可靠，可促进患者康复，具有较好的临床应用价值。 　　【关键词】 UMP； 肾结石； 术后； 应激； 肾功能 　　【Abstract】 Objective：To analyze the changes of postoperative stress and renal function in patients with renal calculi treated with ultra-mini percutaneous nephrolithotomy（UMP）.Method：100 patients with renal calculi in our hospital from April 2015 to October 2016 were selected as the study subjects.They were divided into observation group and control group according to the random number table method，50 cases in each group.The observation group was treated with UMP and the control group was treated with percutaneous nephrolithotomy（PCNL）.The operation time，bleeding，hospitalization time，stone clearance rate and postoperative complications of two groups were compared， the changes of interleukin-6（IL-6），interleukin-10（IL-10），C-reactive protein（CRP），cortisol（COR），serum creatinine（Cr） and blood urea nitrogen（BUN） of two groups were analyzed.Result：Compared with the control group，the operation time and hospitalization time were shorter，the bleeding volume was less，the stone clearance rate was higher，the difference were statistically significant（P0.05）；the levels of IL-6，IL-10，WBC，CRP and COR in the observation group were significantly lower than those the control group at 1， 3 and 5 days after operation，the difference were statistically significant（P0.05）.Conclusion：UMP treatment of renal calculi with shorter operation time，less intraoperative blood loss，high stone clearance rate，less postoperative stress and no obvious effect on renal function，safe and reliable，can promote
