重复经颅磁刺激联合艾司西酞普兰治疗产后抑郁症对照研究 　　【摘要】 目的 观察重复经颅磁刺激（rTMS）联合艾司西酞普兰治疗产后抑郁症的疗效及安全性。方法 80例产后抑郁症住院患者随机分为治疗组和对照组， 各40 例。两组患者均给予艾司西酞普兰治疗， 治疗组在此基础上应用高频rTMS治疗， 疗程4周。在治疗前和治疗后 1、2、4 周末采用汉密顿抑郁量表（HAMD）评定抑郁症状， 采用副反应量表（TESS）评定副反应。结果 共完成77例， 治疗组39例， 对照组38例。治疗组HAMD评分在治疗1周末与治疗前比较， 差异有统计学意义（P0.05）；两组 HAMD 评分在治疗 2 周及4周末与治疗前比较， 差异均有统计学意义（P0.01）。两组均未观察到明显的副反应。结论 rTMS联合艾司西酞普兰治疗产后抑郁症疗效肯定， 起效迅速。 　　【关键词】 重复经颅磁刺激；产后抑郁症；艾司西酞普兰 　　DOI：10.14163/j.cnki.11-5547/r.2015.18.007 　　Control study of repetitive transcranial magnetic stimulation combined with escitalopram in the treatment of postpartum depression LU Guang-yi， ZHANG Qian-nan， YU Dan. Zhumadian City the Second People’s Hospital， Zhumadian 463000， China 　　【Abstract】 Objective To observe the curative effect and safety of repetitive transcranial magnetic stimulation （rTMS） combined with escitalopram in the treatment of postpartum depression. Methods A total of 80 postpartum depression patients were randomly divided into treatment group and control group， with 40 cases in each group. Both groups received escitalopram for treatment， and the treatment group received additional rTMS for 4 weeks. Hamilton depression scale （HAMD） was applied before treatment and in 1， 2， 4 weekends after treatment for depression symptom evaluation， and treatment emergent symptom scale （TESS） was applied for adverse reactions evaluation. Results There were totally 77 completed cases， as 39 cases in the treatment group and 38 cases in the control group. The treatment group had statistically significant differences of HAMD scores before treatment and in 1 weekend after treatment （P0.05）. Both groups had statistically significant differences of HAMD scores before treatment and in 2 and 4 weekends after treatment （P0.01）. There were no obvious adverse reactions in the two groups. Conclusion Combination of rTMS and escitalopram provides precise effect in the treatment of postpartum depression with quick onset. 　　【Key words】 Repetitive transcranial magnetic stimulation； P
