集束化护理在预防呼吸内科重症患者误吸中应用 　　[摘要]目的 探讨集束化护理在预防呼吸内科重症患者误吸的效果。方法 选取2016年1～12月我院呼吸内科重症患者48例作为观察组；2015年1～12月呼吸内科重症患者41例作为对照组，对照组采用常规护理，观察组在对照组基础上采取集束化护理，比较两组患者误吸发生率、住院时间、转ICU治疗率及焦虑水平。结果 观察组患者误吸发生率、转ICU治疗率低于对照组，住院时间短于对照组，差异有统计学意义（P0.05）；护理后两组患者的SAS评分均有所降低，与护理前比较，差异有统计学意义（P0.01）；观察组SAS评分均低于对照组，差异有统计学意义（P0.01）。结论 集束化护理在预防呼吸内科重症患者误吸中具有积极的作用，可有效地减少误吸的发生，显著缩短患者的住院时间，降低转ICU治疗率，减轻患者的焦虑抑郁状态，改善患者的临床结局，值得临床广泛应用及推广。 　　[关键词]集束化护理；呼吸内科；重症患者；误吸 　　[中图分类号] R473.5 [文献标识码] A [文章编号] 1674-4721（2017）06（b）-0194-03 　　[Abstract]Objective To explore the application of cluster nursing in the prevention of aspiration in severe patients in Department of Respiratory Medicine.Methods 48 severe patients from January to December 2016 and 41 severe patients from January to December 2015 in Department of Respiratory Medicine in our hospital were selected respectively as the observation group and the control group.Patients in the control group were given the routine nursing，while patients in the observation group were given the cluster nursing on the basis of the control group.The incidence of aspiration，the length of hospital stays，the incidence of ICU treatment and the level of anxiety were compared between two groups.Results The incidence of aspiration and ICU treatment in the observation group were lower than that in the control group，the length of hospital stays was shorter than that in the control group，and the differences were statistically significant（P0.05）；after nursing， the scores of SAS in two groups were all declined，and compared with those before nursing，the differences were statistically significant （P0.01）.And that in the observation group was higher than that in control group，and the difference was statistically significant （P0.01）.Conclusion The cluster nursing has a positive effect on the prevention of aspiration in severe patients in respiratory medicine，which can effectively reduce the incidence of aspiration，significantly shorten the length of hospital stay，reduce the incidence of IC
