嗜酒和不饮酒银屑病病例临床对比分析 　　【摘要】 目的 研究嗜酒对银屑病病情及治疗的影响。方法 将嗜酒组197例与不饮酒组183例进行临床比较,观察两者之间皮损类型、皮损分布情况、严重程度、复发频度及治疗效果的差别。结果 嗜酒组和不饮酒组比较,皮损类型前者混合型和斑块型病例较多(P0.05);复发频度前者常年不愈病例明显多于后者(P0.01),而能间隔3～4年复发的病例前者明显少于后者(P0.01),能保持5年及以上复发者前者显著少于后者(P0.01)。治疗效果前者不及后者(P0.05)。结论 嗜酒对银屑病的皮损类型、皮损分布情况、严重程度、复发频度及治疗效果均可产生不利影响。 　　【关键词】 银屑病;嗜酒;不利影响 　　【Abstract】 Objective To study the effect of drink abuse on the patients condition and treatment of psoriasis.Methods The type,distribution,severity,frequency of recurrence and therapeutic effect of skin lesions in 197 drinkers(drinking group)and 183 non-drinker(non drinking group)were analyzed and compared.Results Mixed and plaque type of the drinking group were more than those of the non drinking group,while the psoriasis erythem was dominant in non drinking group;Generalized psoriasis in the formergroup was more than that in the later group and localized psoriasis was less than that in the latter group;There were more serious patients and less mild patients in the drinking group.With respect to recurrence,the former group had more patients who relapsed every year,and the patients who relapsed after 3～4 years were obviously less,and the patients who relapsed after 5 years or longer were significantly less than the latter group;The therapeutic effect of the latter group are better than that of the former group.Conclusion Drink abuse have adverse effect on the type,distribution,severity,recurrence and therapeutic effect of psoriatic lesion. 　　【Key words】 Psoriasis;Drink abuse;Adverse effects 　　 　　银屑病的发生、发展及复发常受到许多因素影响,已有的研究初步证实嗜酒是影响因素之一[1]。然而既往研究尚未见嗜酒与不饮酒病例之间临床对照研究的报道。为积累资料,现将我们对一组病例的观察情况总结分析如下。 　　1资料与方法 　　1.1病例选择两组入选病例均为18岁以上男性成年寻常型银屑病患者。病程中未系统使用过糖皮质激素、抗肿瘤药及免疫抑制剂;未合并需长期系统用药的慢性系统性疾病;无重大精神创伤史。嗜酒组病例,限定必须是每月饮白酒总量为1 500 mL及以上者(具体是:每天饮白酒50 mL及以上,或每周不论饮酒次数但总量在350 mL及以上,或每月不论饮酒次数但总量在1 500 mL及以上。每月饮酒总量未达到1 500 mL者不作为观察对象);不饮酒组病例,必须是从来不饮酒者。此外,两组还均除外饮啤酒、红酒及药酒者。 　　1.2病例资料嗜酒组197例,年龄26～69岁,平均45.7岁;病程5～41年,平均24.3年;月饮白酒量1 500～15 000 mL,平均2 850 mL/月;嗜酒历史3～37年,平均11
