孤独症谱系障碍早期社区筛查探索性的研究
[摘要] 目的 探索出一?N切实可行并适用于该地区的孤独症谱系障碍早期社区筛查模式,做到早期发现、早期诊断,以免错过最佳有效干预期。方法 2016年6―10月在福州市台江区某社区儿保门诊对18~24月龄儿童使用Chat-23量表进行孤独症谱系障碍筛查,共有316名儿童参与该次筛查工作。 结果 初筛阳性率为12.0%,其中男童13.4%,女童10.4% ;差异无统计学意义(P0.05)。复筛阳性率为2.2 %,其中男童1.7%,女童2.8%;差异无统计学意义(P0.05)。复筛阳性检出率为25.0%,其中男童12.5%,女童33.3%;差异无统计学意义(P0.05)。复筛率为73. 7%,其中男童70.0%,女童80.0%;差异无统计学意义(P0.05)。结论 在现有的三级妇幼保健工作系统中,使用Chat-23量表进行儿童孤独症的早期筛查,并将此作为儿童保健工作常规项目来开展是可行、有效的;但同时必须加大孤独症知识的宣传和普及,提高基层医务人员相关专业水平及公众的认识程度,从而推动全市乃至于全国范围内孤独症谱系障碍早期社区筛查模式。

[Abstract] Objective This paper tries to explore a feasible and effective early screening pattern on autism spectrum disorders in local communities so as to achieve effective intervention through early detection and early diagnosis. Methods Chat -23 scale was used to conduct autism screening among 316 children aged between 18 months and 24 months from child health care outpatient departments of one community in Taijiang District from June 2016 to October 2016. Results The positive rate of the primary screening was 12.0%, the positive rates of the primary screening in boys and girls were 13.4% and 10.4%, there were no significant difference between the boys and the girls(P0.05). The positive rate of the secondary screening was 2.2%, the positive rates of the secondary screening in boys and girls were 1.7% and 2.8%, there were no significant difference between the boys and the girls(P0.05). The positive detection rate of the secondary screening was 25.0%, the positive detection rates of the secondary screening in boys and girls were 12.5% and 33.3%, there were no significant difference between the boys and the girls (P0.05). The secondary screening rate was 73.7%, among which the boys and girls were 70.0% and 80.0%, there were no significant difference between the boys and the girls (P0.05).
